Skin Biopsy: The "safe" procedure that goes wrong 30 per cent of the time

A skin biopsy is supposed to be one of the simplest, and safest, procedures - and yet it goes wrong in nearly a third of all cases.

When a biopsy is performed, a small slither of skin is removed, under local anesthetic, to determine if a mole or rash, for instance, is something more serious, such as cancer.

But while the patient is assured the procedure is completely safe, researchers have discovered that nearly a third of all biopsies have complications, such as wound infections.

They made the discovery when they tracked the progress of 100 patients who underwent a skin biopsy at a dermatology ward in a university teaching hospital. Wound complications occurred in 29 of the cases, and all but two were caused by wound infection.

The patient was more likely to suffer complications if the skin biopsy was performed below the waist, if the procedure happened in a hospital ward rather than in the outpatient unit, if the patient was a smoker and if he or she was taking corticosteroid drugs at the time.

Public Citizen Sues FDA in Federal Court to Force Agency to Act On Petition Seeking Stricter Fluoroquinolone Warnings

WASHINGTON, D.C. - Despite long-standing evidence that fluoroquinolone antibiotics can cause tendon ruptures, the Food and Drug Administration (FDA) has failed to increase its warnings to patients and physicians about the dangers of the medicines, Public Citizen told a federal court Thursday.

Public Citizen sued in the U.S. District Court for the District of Columbia, asking the court to force the FDA to act upon a petition the consumer group filed with the agency 16 months ago. The FDA has failed to respond to the petition, which asked the agency to put a ?black box? warning on fluoroquinolone antibiotics (such as Cipro, Levaquin and others) to make doctors and patients more aware of the risk of serious tendon injury before tendons actually rupture.

The petition also urged the FDA to send a warning letter to physicians, as well as require an FDA-approved medication guide to be dispensed when prescriptions are filled. Public Citizen contends that the FDA is violating the Administrative Procedure Act by not acting upon the petition.

Stronger warnings could lead to earlier intervention and prevent needless injuries by allowing doctors to switch patients to other antibiotics, said Dr. Sidney Wolfe, director of Public Citizen's Health Research Group.

"While the FDA sits idly by and ignores the problem, more people will suffer serious tendon ruptures that could have been prevented," Wolfe said. "The current warning is buried in a long list of possible adverse reactions and is far too easy to miss."

From November 1997 through December 2005, the FDA received 262 reports of tendon ruptures, mainly of the Achilles tendon, 258 cases of tendinitis and 274 cases of other tendon disorders in patients using fluoroquinolone antibiotics. An additional 74 tendon ruptures have subsequently been reported to the FDA for a total of 336. Because only a small fraction of cases are typically reported to the FDA, the actual number of ruptures and other tendon injuries attributable to the antibiotic is much higher.

Carbon Dioxide Pollution Kills Hundreds a Year - Study

WASHINGTON - Climate-warming carbon dioxide spewed by coal-fired power plants and fossil-fueled vehicles has been causing hundreds of premature US deaths each year over the several decades, a new study reported.

The deaths were due to lung and heart ailments linked to ozone and polluting particles in the air, which are spurred by carbon dioxide that comes from human activities, according to the study's author, Mark Jacobson of Stanford University.

As the planet warms due to carbon dioxide emissions, the annual death rate is forecast to climb, with premature deaths in the United States from human-generated carbon dioxide expected to hit 1,000 a year when the global temperature has risen by 1.8 degrees F (1 degree C).

When the planet gets that hot, which could happen this century, the world annual death rate is estimated to rise to 21,600, Jacobson said on Friday in a telephone interview.

Earth has warmed about 1.4 degrees F (0.8 degrees C) in the last 150 years, with most of that gain in the last three decades. Jacobson said about 700 to 800 US annual deaths in the most recent years can be attributed to human-caused carbon emissions.

Greenhouse gas pollution has spurred the global warming that is result in a damaging rise in the sea level, droughts and possibly more severe storms this century. This is the first time a scientist has specifically linked one human-generated greenhouse gas to human mortality.

Carbon dioxide is one of several greenhouse gases blamed for climate change, but it is the one humans have the most ability to control through regulation of activities that burn fossil fuels like coal and oil. It is also emitted by natural processes.

IMPACT ON CALIFORNIA

Using a complex computer model and data on carbon emissions from the US Environmental Protection Agency, Jacobson found the impact was worse in places that are populous and polluted.

"Of the additional ... deaths per year due to ozone and particles ... about 30 percent of those occurred in California, which has 12 percent of the (US) population," he said, noting that California has six of the 10 most polluted US cities.

"So it was pretty clear ... that climate change was affecting Californians' health disproportionately to its population," Jacobson said.

What happens in California is important, since this populous state has long been a testing ground for US pollution regulation.

Jacobson's study, to be published in Geophysical Research Letters, was released soon after the US Environmental Protection Agency rejected a bid by California and 15 other states to let them set higher standards for carbon emissions from cars, trucks and SUVs than the federal government does.

Jacobson's research was not available before the EPA's decision on Dec. 19, but the EPA's rejection made points that Jacobson said are addressed by his study.

In turning down the states' request, EPA argued that California did not have a special circumstance warranting this change, that there were no studies isolating carbon dioxide's effects and none looking at health impacts.

"It's actually occurring right now, it's been occurring for the past 20 to 30 years," Jacobson said of the deaths related directly to human-generated carbon dioxide emissions.

He noted, however, that the deaths due to carbon dioxide are only a small fraction of annual premature deaths caused by air pollution overall: an estimated 50,000 to 100,000 in the United States and between 1.5 million to 2 million worldwide.
