US sugar barons 'block global war on obesity'

Leading scientists accused the Bush administration last night of putting the interests of powerful American sugar barons ahead of the global fight against obesity.
Professor Kaare Norum, leader of the World Health Organisation's fight to prevent millions developing diet-related diseases, has sparked an international war of words with a highly critical letter to US Health Secretary Tommy Thompson. In it he tells of his grave concern over American opposition to the WHO's blueprint to combat obesity. He accuses the US of making the health of millions of young Americans 'a hostage to fortune' because it has failed to take action over the fat epidemic as a result of its business interests, particularly the sugar lobby.

Since 1990, successive US governments have blocked WHO calls for action, claims Norum, professor of medicine at Oslo University.

'Obesity rates have risen so that now one in three Americans bears the burden of the very high health risks associated with this condition, with the poorest and most vulnerable worst affected,' he says. 'Obesity rates among American children have risen by 50 per cent.'

Norum is the most senior scientist involved in an attempt to formulate a worldwide policy to fight heart disease and diabetes resulting from a junk food diet. An estimated 60 per cent of disease worldwide is now due to cardiovascular illness, which causes 47 per cent of deaths.

The letter from Norum will put Bush under intense pressure at home to show that he is serious about tackling the epidemic. More than half of all Americans are overweight, and in some states, including Bush's Texas, nearly one-third of the population is classified obese.

The President insists fighting fat is a matter for the individual, not the state. But today The Observer reveals how he and fellow senators have received hundreds of thousands of dollars in funding from 'Big Sugar'. One of his main fundraisers is sugar baron Jose 'Pepe' Fanjul, head of Florida Crystals, who has raised at least $100,000 for November's presidential re-election campaign.

Norum's letter is an angry response to the Americans' decision to submit a 30-page report, criticising the WHO strategy for its lack of sound scientific evidence. It will be discussed at a key meeting of its executive board in Geneva on Tuesday.

The Bush administration, which receives millions in funding from the sugar industry, argues there is little robust evidence to show that drinking sugary drinks or eating too much sugar is a direct cause of obesity. It particularly opposes a recommendation that just 10 per cent of people's energy intake should come from added sugar. The US has a 25 per cent guideline.

Thompson's representative at Tuesday's meeting will be Bill Steiger, godson of George Bush Sr. He will argue there is no evidence that selling junk food to children increases overweight.

Another leading obesity expert supported Norum, describing America's position as a scandal. Professor Philip James, head of the International Obesity Task Force, a thinktank for experts worldwide said: 'People are far more tuned into what is now a much bigger obesity crisis and are more aware of some of the dangers such as diabetes. When they begin to see children developing these severe health problems, it brings home to people that this is not some vague risk in the future - it is happening here and now.'

Thompson is also due to speak at the World Economic Forum in Davos next week, where he is expected to have a private meeting with Douglas Daft, president of The Coca-Cola Company, one of the major users of American cane sugar and sweeteners.

Antibiotics Not The Answer For Heart Disease Prevention

In recent years, some heart disease researchers have been
exploring the relationship between heart attacks and bacterial
infections. Plenty of evidence exists to support such a
relationship. It appears that those who test positive for
infection by ba cteria called Chlamydia pneumoniae have elevated
risk of coronary artery disease. A relationship between gum
disease (which is caused by bacteria) and heart attacks has also
been found in several studies. Interesting, and worthy of further
exploration­but with all of the bad press that's been coming out
about antibiotic overuse, testing long courses of antibiotics in
the prevention of heart disease would seem out of the question,
right?

Wrong! A research team at Duke University just completed a large
study of an extended course of azithromycin (3 months) for the
reduction of risk of death from heart disease in the two-year
period following a heart attack. Just under 7,500 heart attack
patients from the U.S., Argentina, and India participated in the
study. All subjects had had a heart attack at least six weeks
prior to entering the study, and all tested positive for
C. pneumoniae.

After a median of 14 months of follow-up­during which patients
were tracked to see whether those on antibiotic therapy were less
likely to die, have another heart attack, or require bypass
surgery or hospitalization for angina­it became evident that the
antibiotic therapy was making absolutely no difference. About 13
percent of those on azithromycin developed side effects, mostly
in the form of diarrhea.

I'd like to see a similar study using live probiotic bacteria,
the body's natural defense against "bad" bacterial infections.
People who have high heart disease risk are, in my estimation,
likely to have low probiotic levels. Reestablishing the balance
of healthy bacteria rather than obliterating both good and bad
with antibiotics makes more sense in the long run.

 Making Sure Children Slumber Soundly
Simple test identifies those with sleep-disordered breathing

FRIDAY, Jan. 16 (HealthDayNews) -- A simple, noninvasive test that can be done while children are awake may help identify youngsters with sleep-disordered breathing.

The good news comes from a study in the January issue of the American Journal of Respiratory and Critical Care Medicine.

The study examined 247 children between the ages of 4 and 16 who snored. Researchers measured the children's upper airway cross-sectional area, a noninvasive procedure that lets doctors identify airway problems that result in disordered breathing.

The children were first given a topical anesthetic in the upper airway. The anesthetic relaxes local reflexes that promote airway openness when a person is awake.

When they used this test on the children, the researchers found that those with sleep-disordered breathing had smaller airways and that both the upper airway collapsibility and sleep-disordered breathing severity were "markedly" and "significantly greater" in such children.

Snoring is a major symptom of sleep-disordered breathing, which affects about 11 percent of children aged 2 to 8. But diagnosing the disorder usually requires an overnight sleep test, which can be a major burden for children and parents.

Another study in the same issue of the American Journal of Respiratory and Critical Care Medicine found continuous positive airway pressure (CPAP) treatment rapidly improves insulin sensitivity in people with obstructive sleep apnea syndrome (OSAS).

Alaska Thaws, Complicating the Hunt for Oil

DEADHORSE, Alaska — Harry Bader slogged across a patch of America's only Arctic shore, leaning into a late December gale that filled the midday twilight with blowing snow and sent the wind chill to 40 below.

Despite the weather, Mr. Bader, the state's land manager for the oil-rich North Slope, was consumed with one thing — the warming climate. Oil-prospecting convoys in search of new deposits are allowed to crisscross the fragile tundra only when it is snowy and solid. But over three decades, rising temperatures have cut this frozen season in half, to 100 days from 200.

Environmentalists have begun to point out the contradictions in a situation where Arctic-wide warming, which many scientists say is at least partly driven by smokestack and tailpipe emissions, is curtailing the quest for a fossil fuel that is a prime source of such pollution. Nowhere is the warming trend more acute than here on this Minnesota-size stretch of pond-pocked plains and shrubby foothills.

But even as temperatures have risen, so has the demand for oil from the Slope, which holds the country's last big known domestic reserves. And Mr. Bader's agency, the Department of Natural Resources, is bound by the State Constitution not only to protect the land, but to increase extraction of the oil and gas beneath it.

Something had to give — and the only flexibility lay in the rules for tundra travel. So, with $270,000 from the federal Department of Energy and the help of the oil industry, Mr. Bader was out on the frigid plains testing a new way to measure the landscape's ability to withstand the impact of oil exploration.

He hiked through drifts by the light of a headlamp, leading a Caterpillar bulldozer, a Tucker Sno-Cat and other machines in Figure 8's to measure their effect on the vegetation beneath the snow. After repeating this ritual every two weeks through the winter, he will return in the summer to assess any damage.

Mr. Bader was unabashed in saying that his goal was to give oil companies and state and federal officials what they wanted, an expanded oil-hunting season, by replacing what he called an overly protective rule with something more precise.

For decades, officials have relied on a simple test to determine when the spongy soil and the carpet of lichens, sedges and other vegetation were shielded from scarring by the heavy vehicles. Six inches of snow and a foot of frozen ground — measured by hammering a finger-thick steel bar into the earth — meant the convoys could roll. With those rules, the exploring season had dwindled.

All of this was happening as the Bush administration, Gov. Frank H. Murkowski, the oil industry and Mr. Bader's agency were pushing more than ever to expand oil exploration. Established wells were playing out and the Senate still refused to allow the oil hunt to expand into the Arctic National Wildlife Refuge, the most familiar part of the North Slope.

"We have to find out what the standard really ought to be," Mr. Bader said. "When the season was 200 days a year, who cared if we were off by 40 days? But we're right on that cusp of the minimum number of days now."

The oil industry says it needs 120 days a year to avoid having projects drag out expensively. "We still want to protect the Arctic environment," said Larry Houle, general manager of the Alaska Support Industry Alliance, a trade group for oil subcontractors. But because of the shrunken season, he said, "if you discover oil today it's going to take you eight years to get it developed."

Mr. Bader said he was braced for attack, knowing that environmental groups were already saying any rule revision would inevitably amount to a relaxation of protections. And his work is being watched by the Bush administration, which recently announced that it planned to open to oil development a new area of 8.8 million acres of federal lands at the western side of the Slope. The only way for oil companies to get there is to cross state lands, where access is increasingly constrained by the tundra rules.
