Traces of
Prescription Drugs Found in Southland Aquifers

Various medications are detected in drinking water that has been derived
from treated sewage. The health risk, if any, is unknown.
Behind a tangle of willows, every second of every day for almost half a
century, recycled sewage has gushed into an El Monte creek and nourished
one of Los Angeles County's most precious resources: the drinking water
stored beneath the San Gabriel Valley.
Cleansed so thoroughly that it is considered pure enough to drink, this
flow from the Whittier Narrows reclamation plant meets all government
standards. Yet county officials now report that they have found some
potent - and until recent months undetected - ingredients in the treated
waste: prescription drug.
As new technology enables detection of infinitesimally smaller doses of
chemicals in the environment, Southern California water-quality
officials have learned that an array of hardy pharmaceuticals are
defying even the most sophisticated sewage treatments in use.
Around the world, waterways and groundwater basins are virtual
drugstores, awash in low doses of hundreds of prescription drugs
excreted by people and flushed down drains.
Wherever there is sewage, there are traces of whatever pills people have
popped: antibiotics and antipsychotics, birth-control hormones and beta
blockers, Viagra and Valium.
"There is no place on Earth exempted from having pharmaceuticals and
steroids in its wastewater," said Shane Snyder, head toxicologist at Las
Vegas' water provider, the Southern Nevada Water Authority, and one of
the nation's leading experts on pharmaceuticals in water. "This is
clearly an issue that is global, and we're going to see more and more of
these chemicals in the environment; no doubt about it."
Locally, small amounts of medicines for depression, seizures, high
cholesterol, anxiety, infections, inflammation and pain - among other
ailments - have been detected in the wastewater that flows into
California streams and seeps into drinking-water aquifers. The
contamination raises questions about the safety of reclaimed water
consumed by the public and the health of wild creatures that inhabit
waterways.
The concentrations are so minuscule - in parts per trillion, or a few
drops in an Olympic-sized swimming pool - that scientists suspect there
is little or no human danger. They acknowledge, however, that no one
knows the effects of ingesting tiny doses of multiple drugs continuously
over a lifetime.
So far, concerns have focused mostly on the ecological threat.
Biologists studying frogs on Prozac, insects dosed with anti-seizure
drugs, algae killed by antibiotics and fish feminized by birth-control
pills have discovered that some streams contain pharmaceuticals and
synthetic estrogen at levels harmful to aquatic life.
"All the data we have compiled indicates these concentrations are
trivial to public health. Even putting massive safety factors on this,
it still wouldn't have a [human] impact," Snyder said. "Now for
wastewater ? that's a different story. When you have a fish or
endangered species that is exposed 24 hours a day, we do need to look at
this."
With thousands of varieties of prescription and over-the-counter drugs
being sold, there are no government standards restricting any of them in
drinking water or in effluent released into streams or lakes.
Water and sewage agencies aren't even required to look for them ? and
most don't. Testing of drinking water for drugs has been so infrequent
that no one knows how much people are ingesting. A national association
of wastewater agencies warned in November that pharmaceuticals are a
"potential sleeping giant."
Los Angeles and Orange counties are among the world's leaders in
recycling sewage to replenish water supplies, and officials there worry
that the public's perception of the water supply will be tainted.
The Whittier Narrows plant, which has operated in El Monte since 1962,
was the nation's first reclamation plant. Since then, nearly half a
trillion gallons of treated sewage from Whittier Narrows and two other
county plants have replenished the Central Basin aquifer beneath the San
Gabriel Valley, which supplies water to 4 million people.
Sewage in Southern California undergoes some of the world's most
rigorous cleansing - tertiary treatment - to protect rivers and streams
from bacteria and nitrogen. Much of the wastewater then is routed into
aquifers, where it remains for at least six months so soil can filter
out more contaminants before potable water is pumped.
In November, the Los Angeles County Sanitation Districts reported at a
scientific conference that they found high levels of ibuprofen, naproxen
and acetaminophen in raw sewage coming into its Whittier Narrows plant,
and very small concentrations going out.
In waste that had undergone treatment, the antibiotic sulfamethoxazole
and anti-cholesterol medication gemfibrozil were found at fairly high
levels of around one part per billion. The antidepressant fluoxetine,
the arthritis drug diclofenac, anti-anxiety and anti-seizure drugs,
three more antibiotics and others were detected at lower levels, in
parts per trillion. Estrogens also were measured in low levels.
Similar findings from two Los Angeles County reclamation plants will be
published later this year by Jorg Drewes, an assistant professor of
environmental science and engineering at the Colorado School of Mines.
Robert Horvath, the districts' technical services director, said tiny
doses of over-the-counter drugs aren't that worrisome, but other less
common medications can amount to an involuntary though "extremely low"
public exposure. The agency, which operates 10 reclamation plants, is
one of a few with the ability to test for pharmaceuticals.
"It's such a large list of compounds that even the testing is a lot of
work - just teasing out which ones are important. So far, we have no
[federal or state] goals to shoot for," Horvath said.

California is the only state with this problem, think again... If you
are on a city water system you are getting the same thing only most
likely worse... Southern California has the top systems in the country
yet not effective... Since so many medications are not metabolized by
the body and is ultimately flushed down the sewer drain when expelled
from the body and sewer plants are so archaic that they do not have the
ability to filter them all out... Starting the round robin effect of
building up in the aquifers, rivers and streams... Another thing this
articles does is give a false sense of safety by saying "scientists

When in Hawaii this particular fruit found its way into my plate on many
occassions and I truly loved finding it there... Something that I had
not eaten until about ten years when friends from Belez introduced me to
them and since that time I eat them as often as possible...
Mango grow at the end of a long, stringlike stem, with sometimes two or
more fruits to a stem. The fruits are 2 to 9 inches long and may be
kidney shaped, ovate or rarely but sometimes round. They range in size
from 8 ounces to around 24 ounces. The leathery skin is waxy and smooth,
and when ripe entirely pale green or yellow marked with red. It is
inedible and contains a sap that is irritating to some people. The
quality of the fruit is based on the scarcity of fiber and minimal
turpentine taste. Mango is regarded as the queen of fruits in tropical
areas of the world.
The flesh of a mango is peachlike and juicy, with more or less numerous
fibers radiating from the husk of the single large kidney-shaped seed.
Fibers are more pronounced in fruits grown with hard water and chemical
fertilizers. The flavor is pleasant and rich and high in sugars and
acid.

The Mango tree plays a sacred role in India; it is a symbol of love and
some believe that the Mango tree can grant wishes. It is also the
world's largest producer of mangoes. It has been estimated that there
are over 1000 commercial varieties in India, where mangos are often
called the "king of fruits".
Florida is the main producer of mangos in the United States. In 1990,
2800 acres of mangos were planted in Florida, of which 2500 acres were
harvested. The farm value of the 19.2 million pounds produced was $4.7
million. Mangos are also produced in Hawaii and Southern California.
There are two principal types of mangos: Indian and Indochinese. ...
Within those two type are; ‘Ah Ping’, ‘Fairchild’, ‘Gouveia’, ‘Harders’,
‘Keitt’, ‘Momi K’, ‘Pope’, and ‘Rapoza’ are recommended mango varieties
for Hawaii. All the listed varieties are productive and have superior
quality fruit. They have less pronounced alternate-year bearing
qualities than the more common ‘Haden’ and ‘Pirie’ varieties. All these
varieties, including ‘Haden’ and ‘Pirie’, are monoembryonic and do not
come true from seed. Flowering occurs from December to April, but
offseason flowering is common, resulting in variable harvest times.
‘Fairchild’ is considered somewhat resistant to anthracnose and is
favored for humid areas.
Mango can be eaten raw as a dessert fruit or processed to various
products. Ripe fruits can be sliced and canned or processed to juice,
jams, jellies, nectars and preserves. Eastern and Asian cultures use
unripe mangos for pickles, chutney and relishes. In India, unripe mangos
are sliced, dried, and made into powder for amchoor, a traditional
Indian preparation used for cooking.In India, flour is made from mango
seeds. Seeds are also eaten during periods of food shortages. The timber
is used for boats, flooring, furniture and other applications.

Back Ban on Gifts From Drug Makers

The gifts, drugs and classes that makers of pharmaceuticals and
medical devices routinely give doctors undermine medical care,
hurt patients and should be banned, a group of influential
doctors say in today's issue of The Journal of the American
Medical Association.
Medical schools and teaching hospitals should be the first to
establish a comprehensive ban, the group writes. But the authors
argue that all doctors should eventually follow suit.
Broadly adopted, the recommendations would transform doctors'
day-to-day lives and shut off the focus of drug makers' biggest
expenditures. But Dr. David Blumenthal, an author of the
article, said it was "not very likely" that many in medicine
would listen to the group.
"I'm not very optimistic," said Dr. Blumenthal, a professor at
Harvard Medical School who, like many of the article's 10 other
authors, has studied conflicts of interest in medicine for
years.
Federal law forbids companies from paying doctors to prescribe
drugs or devices, but gifts and consulting arrangements are
almost entirely unregulated. Voluntary professional guidelines
suggest that doctors refuse gifts of greater than "modest"
value. Sanctions against doctors who accept gifts of great value
are extremely rare.
The drug industry spends tens of billions of dollars a year to
woo doctors, far more than it spends on research or consumer
advertising. Some doctors receive a significant part of their
income from consulting arrangements with drug and device makers.
Others take regular vacations and golfing trips that are paid
for by companies.
A recent lawsuit involving the device maker Medtronic revealed
that one prominent Wisconsin surgeon received $400,000 for a
consulting contract that required him to work just eight days.
While such rich arrangements are often restricted to
specialists, most physicians routinely accept small gifts from
drug salespeople, including pens, mugs, pads and food.
Surveys show that most doctors do not believe that these gifts
influence their medical decisions, although most believe that
they do affect their colleagues' medical judgment.
But even small gifts can lead to profound changes in doctors'
prescribing behavior, with "negative results on clinical care,"
the article states. As a result, all gifts should be banned, the
authors conclude.
Ken Johnson, a spokesman for the Pharmaceutical Research and
Manufacturers of America, said the drug industry had a voluntary
code of marketing conduct.
"Only practices that do not compromise independent judgments of
health providers - such as modest working meals, gifts of
minimal value that support the medical practice, and
distribution of free samples - are permitted," Mr. Johnson said
in a statement.
Dr. Duane M. Cady, board chairman of the American Medical
Association, said in a statement that "drug and medical device
makers can play a role in educating physicians about new
products." He said the organization was "in the process of
examining and updating its policy on gifts to physicians from
industry."
The article is part of a spate of reports in medical journals
that have taken a skeptical view of drug makers' influence on
medical practice and research. The New England Journal of
Medicine recently published an article accusing Merck of
withholding crucial safety information about the withdrawn
painkiller Vioxx, a charge that Merck denies. Other articles
have criticized drug makers' tendency to keep the results of
human research secret.
Dr. Catherine DeAngelis, editor in chief of The Journal of the
American Medical Association, said drug makers were a vital part
of the nation's health care system because of their research
efforts.
"But there has been a substantial change in the way
pharmaceutical companies function over the last decade," Dr.
DeAngelis said. Drug makers are far more aggressive in their
marketing efforts, she said, and these efforts are having a
deleterious effect on the practice of medicine.
Dr. Steven Shea, vice dean of the faculty of medicine at
Columbia University Medical Center, predicted that the journal
article would "prompt changes in policy and guidelines at many
academic health centers, including ours."
Kaiser Permanente, the California-based managed-care group, is
one of the few medical organizations in the United States that
have enacted nearly all of the recommendations suggested by the
journal article. Kaiser physicians prescribe heavily marketed
medicines far less frequently than doctors nationally.
"We thought it was critical for us that our patients never had a
doubt that the decision made about a drug or a device was based
on the best interests of the patient and not the financial
interest of the physician," said Dr. Sharon Levine, associate
executive director of Kaiser Permanente Northern California.
The article also argues that "no strings attached" consulting
arrangements should be banned, and that all other consulting
agreements should be posted on Web sites. Doctors should refuse
free drug samples, the article states, because they are "a
powerful inducement for physicians and patients to rely on
medications that are expensive but not more effective."
Such a refusal would also eliminate one of the principal reasons
for which drug salespeople are routinely allowed to enter
doctors' offices, the article states. While the article does not
suggest that salespeople be refused entry into offices, it
states that such visits have few useful functions.
"Would we be delighted if drug reps never saw the inside of
doctors' offices? Absolutely," said Dr. David J. Rothman,
president of the Institute of Medicine and one of the article's
two principal authors. "But you can't mandate that. It's a free
country."
Two years ago, Dr. Rothman received a $7.5 million grant from
the financier George Soros to set up an organization that would
study medical professionalism. Today's article is in part an
outgrowth of that grant, he said.
Dr. Troy A. Brennan, former chairman of the American Board of
Internal Medicine and the other principal author of the article,
said he was looking forward to reading responses to it.
"I don't think there are a lot of good answers as to why it's
O.K. to accept these gifts and contracts," Dr. Brennan said.

Deer Muscle a Source of
Infectious Prions

NEW YORK - Findings from an animal study suggest that disease-cause
prions can be spread via infected skeletal muscle from deer with chronic
wasting disease (CWD) -- a wildlife illness related to mad cow disease.
Whether CWD can be passed to humans is still unclear, but the present
findings suggest that if this does occur, simply handling the meat of
infected dear could pose a risk, senior author Dr. Glenn C. Telling,
from the University of Kentucky in Lexington, and colleagues warn in the
journal Science.
The researchers injected muscle or brain extracts from CWD-affected mule
deer into the brains of mice.
As expected, the brain extracts were much more efficient at causing
disease in the mice. Still, all of the muscle extracts caused
progressive neurologic dysfunction too, albeit with longer incubation
times.
These results show that muscle, "which is likely to be consumed by
humans, is a significant source of prion infectivity," the authors
write. "Humans consuming or handling meat from CWD-infected deer are
therefore at risk to prion exposure."
