Yesterday's Ask Lena Health Q & A was
distributed early but for some reason my service wasn't on the ball and
it didn't go out for several hours after I sent it to the server? Such
is the computer world...
Spring has sprung all over here even if it isn't
official. I was walking out toward the work shop yesterday, getting my
daily sun quota, when I smelled this delicious floral smell and thought
of cherry blossom time in the East, then I looked over toward our small
orchard and saw several fruit trees blooming, hence the floral smell. This all in two days time...
Gorgeous and smells great. I'm looking out my office window and
surveying all
the purple, yellow, white and red blooms on the wild natural flora throughout
our 2.5 acre parcel... Makes working concentration very difficult ... I
know some of you are still suffering cold and snow and I'm sorry, but
just know that won't last forever as it was only three days ago when it
was cold and icy rainy with snow showers now spring flowers and
warmth...

Residents Help Town MD Get License Back

ELGIN, N.D. (AP) - The authorities say that George Hsu wasn't a good
doctor - that a patient died because he delayed care, that he would not
refer others for new treatments he considered a waste of time and money.
They took away his license. But folks in this small town accept none of
that. A committee of 30 people is seeking ways to restore Hsu's license.
More than 300 from southwest North Dakota and northwest South Dakota
gathered for a ham dinner in January, raising thousands of dollars for
his legal bills. To them, he is a well-regarded physician in a rural
area where doctors are scarce and people want to know whom they're
dealing with.
"Everybody is looking for something to do for Dr. Hsu," says Marie
Klein, the committee's leader. "We need him. We trust him."
And Hsu, himself, is unrepentant.
"People here know what I do," he said, "and they don't have a problem
with what I do."
The state Board of Medical Examiners does have a problem with Hsu. Last
March, it suspended the license of the 62-year-old doctor, and in
November it revoked his license, saying he provided inappropriate
patient care in 10 cases.
John Olson, the medical board's attorney, told the panel that the
revocation "is absolutely essential in this case ... he needs to be out
of the practice of medicine."
Hsu has gone to court to file an appeal of the board's ruling, but there
is little reason to believe he will succeed: Medical board executive
secretary Rolf Sletten said that in his 22 years with the board, a judge
has never reversed the panel's decision.
Olson said he could not discuss details of the case against Hsu, but,
"we continue to stand by the record of inappropriate care."
Hsu, not surprisingly, disagrees.
He cites one of the cases for which he was criticized by the board, that
of a man who was brain dead after suffering from heat stroke.
"I told his mom that he was going to die and if we transferred him to a
Bismarck hospital, he was going to die there, too," Hsu said. "Allowing
his mother to be with him was all that I could do. And to me, that's an
honorable thing."
To Hsu, the only one who would have benefited from the man's transfer
would have been the hospital, which would have made thousands of
dollars.
Hsu said he tries to keep patients' costs down. "I know what it's like
not to be able to afford medical treatment. I was a farmer," he said. He
portrays himself as one of the last doctors who work for patients, not
for corporately owned clinics or hospitals.
To those who say Hsu resists change, he responds that he trusts
traditional medical techniques over technology that is not necessarily
for the benefit of the patient.
"I'm not giving the people all the bells and whistles, and people on the
board think that's bad medicine," he said.
Hsu gets hugs and pats on the back when he walks around Elgin, a town of
about 660.
On a recent day, Hsu rushed from a cafe to tend to a woman who had
collapsed at the entrance. He spent several minutes kneeling beside her,
finally helping her to her feet.
"I'm just acting as a concerned citizen, not a doctor," Hsu said.
Hsu estimated that he has seen some 8,000 patients during his 20-year
practice in the region. He had a toll-free telephone number installed at
his home, and was summoned day and night for everything from delivering
babies to mending victims of farm machinery accidents.
He made house calls, he said, and rarely earned more than $40,000 a
year.
Hsu said he was born in China and moved to the United States when he was
6 years old. He cites a background in the military as well as in
medicine: an engineering degree, a commission from the Army and service
in Vietnam.
He moved to Solen, in south central North Dakota, in 1972, to raise
cattle, wheat and a family. Hsu said he remained in the Army Reserve and
served as a doctor in Iraq during the Persian Gulf War. His wife, Kathy,
said she was a military nurse with him in Iraq; they have nine children,
ages 11 to 38, and four of them still live at home.
The couple still own the clinic in Elgin, and they have hired part-time
doctors to run it now that Hsu can't. They closed their clinic in Glen
Ullin, about 30 miles away, in February.
"He's never had a malpractice suit," Kathy Hsu said. "He's good at what
he does, whether he's my husband or not. I don't know what this
community will do without him."
Jill Friesz, publisher of Elgin's weekly newspaper, called Hsu a "great
doctor" but a "renegade" who has had opportunities to change his ways.
"I would like to see him come back," Friesz said. "A lot of old people
here are refusing to seek medical care if they can't see him."
Hsu and his wife said he had an offer five years ago to work for a
hospital on the East Coast, where they would have been closer to
relatives. "There would have been no calls, no weekends and big money,"
Kathy Hsu said.
The local newspaper did a story about Hsu's plans. "People begged him to
stay," his wife said, "and he did." He moved their relatives to North
Dakota, instead.
Now, Hsu's neighbors are fighting for his practice once again. But it's
been almost a year since Hsu saw a patient in his office, and his
calloused hands show it. His knuckles are scraped and his fingernails
are dirty from doing farm work and wrenching on old cars.
"I've always had blue collar hands. Now, they're just worse," he said.

Carambola, Belimbing (Malaysia), Carambolier
(French), Ma Fueng (Thailand), Fuang (Laos), Khe (Vietnam) Averrhoa
carambola, AKA Star fruit belonging to the Oxalidaceae family.
Can be slightly sweet or very sour depending on which species you
choose. A strange looking, bumpy, unique, waxy, golden yellow to green
fruit that has a 5 pointed star shape when cut across the middle of the
fruit, ranging from 3 to 5 inches long with four to six deep lengthwise
grooves, is popular in many countries. When it is sliced crosswise, the
slices are shaped like stars, thus its common name, star fruit. Citrus
nutrients similar to other exotic citrus!
Star fruit of tart and sweet varieties.
Tart varieties generally have narrowly spaced ribs;
sweet varieties have thick, fleshy ribs. Originally from Sri Lanka
and the Moluccas, and cultivated in Southeast Asia and Malaysia for
several hundred years. An excellent source of vitamin C, supplying 35%
of the Daily Value in 3 1/2 ounces, which is equal to a small whole
fruit or about 2/3 cup sliced. I was unable to find more distinct
vitamins and minerals listed in any of the University extensions that
usually list those things but the statement; "Nutrients similar to other
citrus fruits"...
Handle with care!
They bruise easily. Star fruit (the sweet variety) can be eaten out
of hand or sliced and used as a garnish or in salads. They are also used
in chutney, curries and tarts. The juice can be used in tropical drinks
and smoothies. Star fruit is easy to prepare, because it does not
require peeling or seeding. It can be eaten out of hand after it has
been washed. Spray with vinegar and wash under cold running water. Cut
it crosswise into thin slices. If the skin is unappealing, though it
doesn't affect the tatste, simply peel it off with a paring knife.
Ripe fruit should be stored at room temperature for
two to three days or refrigerated, unwashed, in a tightly covered
container or plastic bag for up to one week.
Healthy properties are said to,
reduce fever and cough in common cold; relieve kidney and bladder
stones, shrink hemorrhoids and relieve painful urination and
hemorrhoids. It is claimed that eating the fruit reduces blood sugar
levels in diabetics. In Chinese medicine, they are used as a diuretic.
WARNING: persons in dialysis should take care when eating star fruit as
some cases of intoxication, insomnia and hiccups to agitation, mental
confusion and - in one case death - two people went into renal (kidney)
failure after eating star fruit . Both recovered four weeks later after
hemodialysis… Also do not eat on an empty stomach or if in dehydrated
state as the star fruit may pose an additional risk for development of
renal injury.
Star Fruit Salad
Ingredients
2 star fruit
2 kiwis
2 bananas
1 cup mango fruit nectar
1 cup low fat vanilla yogurt
Makes 4 servings.
Peel kiwi and banana, cut into medium size pieces.
Slice star fruit into ¼-inch thickness. Combine all fruits in bowl. Add
nectar over mixture. Refrigerate for 3 hours. Top with vanilla yogurt.
Drinking the juice alone and unmixed with other
fruits or vegetables not recommended!
Two people – age not given - are reported to have developed nausea,
vomiting, abdominal pain, and backache within hours of ingesting large
quantities of sour carambola juice; then acute renal failure followed
that required dialysis.

Sweetener in the Spotlight: Is Splenda Safe?
Lawsuits Put New Focus on Splenda and Other Artificial
Sweeteners

Feb. 16, 2005 -- Courtroom battles between the makers of Splenda
and Equal have many questioning the safety of artificial
sweeteners.
Since early 2000 McNeil Nutritionals has been advertising that
its product -- Splenda -- is "made from sugar so it tastes like
sugar." But the National Sugar Association and Merisant
Worldwide (maker of Equal brand sweetener) have challenged that
claim in a lawsuit.
McNeil Nutritionals shot back with a countersuit implying the
case against them was more about corporate sour grapes than
truth in consumer advertising.
But court battles and corporate backstabbing aside, the question
on consumers' minds is not so much whether advertising slogans
are right or wrong, but do they really make a difference -- at
home, on the dinner table where it really counts?
Dietitian Nancy Restuccia, MS, RD, says they most definitely do.
"Splenda is not sugar -- and to piggyback it on to the
reputation of the centuries' old profile of sugar is more than
misleading, it could come back to haunt us, perhaps sooner than
we think," says Restuccia, a nutritionist at the Center for
Obesity Surgery at New York Presbyterian/Columbia University
Medical Center in New York City.
Indeed, while there are currently only a handful of studies that
question Splenda's safety and more than 100 which attest to its
safe use, Restuccia says it simply hasn't been around long
enough to amass any long-term data -- or even short-term data
involving heavy consumption.

F.D.A. Seizes Millions of Pills From Pharmaceutical
Plants

WASHINGTON, March 4 - Angered by quality-control problems it said had
dragged on for more than two years, the Food and Drug Administration
used armed federal marshals to seize millions of tablets of two
medicines from facilities in Tennessee and Puerto Rico operated by
GlaxoSmithKline, the agency said Friday.
The drugs are the antidepressant Paxil CR, which had $725 million in
sales last year and is used by some 450,000 patients in the United
States each month; and Avandamet, a diabetes medicine, whose sales are
undisclosed but are far smaller.
The F.D.A. said that neither pill was medically necessary and that many
alternatives existed for both. It added that it knew of no patients
harmed by the poorly made pills and said patients could safely take any
pills they had left.
Officials at both the agency and GlaxoSmithKline said they could not
predict when or how the manufacturing problems would be resolved, though
the company said it satisfied the agency's concerns about Paxil CR last
November.
Despite the absence of evidence that the pills had harmed anyone, the
agency said, a drug maker must be able to assure the public that its
products are properly made.
"F.D.A. and the Department of Justice will not allow drug manufacturers
to ignore our high public health standards for drug manufacturing," said
John M. Taylor, the agency's associate commissioner for regulatory
affairs.
GlaxoSmithKline's shares slid $1.59 Friday, to $48.91, a decline of 3.15
percent.
The problems were detected when inspections beginning in February 2002
at the plant in Cidra, P.R., revealed that Paxil CR tablets tended to
split, leaving patients with half a tablet with no medicine and another
half with no buffering substance.
Avandamet is a combination of two diabetes drugs, Avandia and metformin.
GlaxoSmithKline could not guarantee the potency of the Avandia portion,
the agency said.
Officials said the problems persisted despite an F.D.A. warning letter
and more inspections. The company's inability to figure out what was
going wrong was worrisome, they said.
Nancy Pekarek, a GlaxoSmithKline spokeswoman, said the company believed
that it had resolved its dispute with the drug agency by recalling many
of the pills made before November and inspecting those made afterward.
Paxil CR is a "controlled release" version of the antidepressant. Many
analysts say GlaxoSmithKline created it to fight off generic competitors
to Paxil in the United States, where the CR version is almost
exclusively sold. The company said that version was easier to tolerate
than Paxil.
Still, Dr. James McGough, a professor of psychiatry at the University of
California, Los Angeles, said, "Most patients who are currently
receiving Paxil CR could easily switch to generic versions of Paxil
without any loss of benefit."
Ms. Pekarek said that all "bi-layer" tablets like Paxil CR tended to
split and that the company's new inspection process ensured that damaged
tablets were not shipped to pharmacies.
F.D.A. officials and a top drug manufacturing consultant dismissed that
explanation, saying many companies were able to make bi-layer tablets
that are not prone to splitting. They added that inspections were
unlikely to catch all the damaged pills.
"You cannot just say you'll pick out the bad ones," said a top former
GlaxoSmithKline manufacturing executive, speaking on condition of
anonymity because he now consults for drug companies. "We're not making
strawberries here."
Schering-Plough, Eli Lilly, Wyeth and Abbott Laboratories all have run
afoul of F.D.A. manufacturing rules in recent years. Schering-Plough
paid a $500 million fine in 2002.
"We felt that we have been working with the firm for a couple of years
now, and although they have made some progress in correcting the
problems we felt that without our intervention we couldn't get them to
where they needed to be," said an F.D.A. official, who spoke on
condition of anonymity.
Ms. Pekarek said the problems with the pills were unlikely ever to hurt
patients. Patients who took an inert half of Paxil CR would be no worse
off than if they had simply skipped a day's dose - something that
happens often, she said.
But Dr. Jeffrey Lieberman, a professor of psychiatry at Columbia
University, said Paxil was the wrong drug to skip for a day. Paxil
remains in the bloodstream far shorter than Prozac.
LENA'S COMMENT: Not that this shouldn't have happened but OOPS,
what did they do to the FDA to get this kind of reaction? Maybe the
payoff wasn't large enough? Maybe their scientist refused to go along
with the FDA? Or is the FDA trying to get back on the good side of the
public? Did they fail to have a spokesperson as part of the FDA board?
Lots of questions but no answers yet. The statement "The F.D.A. said
that neither pill was medically necessary and that many alternatives
