Drug-Resistant Bacteria Persist in Chicken

BALTIMORE (AP) - Antibiotic-resistant bacteria continued to be found in
chickens bought at area supermarkets a year after two large poultry
producers stopped using an antibiotic blamed for creating the resistant
strains, Johns Hopkins researchers report.
The researchers say the findings suggest antibiotic-resistant bacteria
may persist in the poultry industry after the use of the antibiotics,
known as fluoroquinolones, has stopped and may contaminate more poultry
than previously thought.
However, one of the producers and a researcher not involved with the
study said it did not show whether the amount of bacteria found
presented a health risk. Whether the resistant strains were naturally
present or use of the antibiotic caused the resistant strains to be
present in the samples also was not clearly shown, the two said.
In 2000, the Food and Drug Administration proposed banning the two
fluoroquinolones used in poultry. Abbott Laboratories of Abbott Park,
Ill., agreed immediately to pull its version, Sara Flox, off the market,
but Pittsburgh-based Bayer Corp. is appealing the decision and the FDA
commissioner is considering the case involving Bayer's product, Baytril.
The bacteria, campylobacter, is responsible for 2.4 million cases of
food-borne illness per year in the United States, according to the
Centers for Disease Control and Prevention. The antibiotic-resistant
form of the bacteria is especially troubling because the fluoroquinolone
family of antibiotics includes the popular drug Cipro, and
fluoroquinolones are a leading treatment for food poisoning from
campylobacter, found mostly in raw chicken.
In February 2002, Perdue Farms Inc. of Salisbury, Md., and Tyson Foods
Inc. (TSN) of Springdale, Ark., stopped using fluoroquinolone
antibiotics for flock-wide treatment.
The researchers said they bought chicken produced by Perdue, Tyson, and
two antibiotic-free producers from Baltimore-area supermarkets between
February 2003 and May 2003, buying three packages each a total of seven
to eight times.
The researchers used two different methods to test for the bacteria. In
one, samples were placed in a growth medium to see if colonies of the
bacteria would form. In the other, the medium contained fluoroquinolone,
which the researchers said enhanced the sensitivity of the testing for
resistant bacteria.
Using the plain growth medium, campylobacter was detected on 84 percent
of the chicken tested, and fluoroquinolone-resistant strains were
detected on 17 percent. When the fluoroquinolone medium was used,
resistant strains were detected on 40 percent of the chicken tested.
By brand, resistant bacteria were found in 33 percent of Tyson chicken
sampled, 96 percent using the fluoroquinolone medium; and 19 percent of
Perdue chicken, 43 percent using the antibiotic medium.
Levels in the two antibiotic-free producers were much lower. Resistant
bacteria were found in 13 percent of samples from one producer using
either testing method, and in 5 percent from the other producer using
the antibiotic medium. Resistant bacteria were not found using the
regular medium in that brand.
"I think the message to the consumer is that they should choose their
brand of chicken carefully," said study author Lance Price. "And they
should consider the use of clinically important antibiotics and
antimicrobials in food animal production to be a potential threat to
their safety."
Tyson noted the sample size was small and limited to one area of the
country, did not measure the amount of bacteria present, and included
species of campylobacter that may be naturally resistant to the
antibiotics without having been exposed to them.
Price said if natural immunity was responsible, "you would expect all
the products to come out the same, but that's not the case."
Perdue said it did not have any information on the origin of the
samples, and "cannot comment on the conclusions regarding antibiotic
resistance."
Perdue said less than 1 percent of its flocks receive any antibiotics,
which are limited to the "humane treatment of ill or at-risk chickens,
treating as few birds as possible, and prescribing that treatment no
longer than deemed medically appropriate by a poultry veterinarian.
"Perdue does not use antibiotics for the purpose of growth promotion nor
do we use any antibiotics continuously for any reason whatsoever,"
according to a statement the company issued.
Randall Singer, an assistant professor of epidemiology at the University
of Minnesota, said the study did not show how much bacteria was present
in each sample, and thus did not show whether the bacteria presented a
health risk.
Singer, a member of two FDA panels on campylobacter and fluoroquinolone
use in poultry, said other studies have shown the resistant strain is
more successful, even when antibiotics are not present, and therefore
could be more likely to be present naturally.
"Clearly, there is a background level of fluoroquinolone-resistant
campylobacter, and when you want to compare retail meat products,
especially on a small scale like this study, there could be many
reasons," Singer said.

studied the nutrient value of different foods. After all, it's not

Among other conclusions, the study showed that although spinach is rich
in beta-carotene, folate and lutein, very little of these nutrients gets
to your blood stream.
The researchers then broke up the plant cells with a blender, and found
that most of the nutrients were released. The blood levels of folate
from chopped spinach were 14 times as high as from leaf spinach, and the
levels of lutein went up 10 times. For some reason that wasn't
explained, the beta-carotene levels didn't change at all.

FDA Issues Warning on Drug Use by Elderly

WASHINGTON (AP) - Elderly patients with dementia were significantly more
likely to die prematurely if taking certain anti-psychotic drugs, the
government said Monday in an advisory to health care workers and
patients.
The Food and Drug Administration is asking manufacturers of atypical
anti-psychotic drugs to add to their labeling a boxed warning noting the
risks and that the drugs were not approved to treat symptoms of dementia
in the elderly.
Elderly patients taking the drugs for dementia-related symptoms should
consult with their doctors, the FDA said.
The drugs were approved for treating schizophrenia and mania, and
include such brands as Abilify, Zyprexa, Seroquel, Risperdal, Clozaril
and Geodon. Symbyax, which is approved for the treatment of depression
associated with bipolar disorder, is also included in the advisory.
An analysis of 17 studies covering four drugs showed the rate of death
for the elderly patients taking them was about 1.6 to 1.7 times the rate
of death for placebo users. The causes of death varied, but most seemed
to be either heart-related or from infection.
The FDA doesn't have data to indicate how many elderly patients take
this class of drugs, but agency officials believe that such
prescriptions are common.
Carole Copeland, spokesman for Eli Lilly and Co., the maker of Zyprexa,
said the company contacted doctors last year to alert them to the higher
risk of mortality cited Monday by the FDA. She said the company also
changed the drug's label last year to reiterate those higher risks. The
company will review the FDA's advisory to determine if further changes
were needed, she said.
Bristol-Myers Squibb and Otsuka America Pharmaceutical, Inc., which
market Abilify, said the companies take the FDA's request seriously and
would respond within 30 days. The companies also said a warning was
added in February to the package insert accompanying Abilify, which
noted a risk of stroke and similar events associated with its use by
elderly patients with dementia.

CHOOSING ASPARAGUS: Select asparagus with tightly bunched tips
(pass if the tips are loose), and minimum of woody white stalk at the
bottom. Wrinkled or pithing stalks indicate age and dehydration and the
taste may not be as good. When asparagus is fresh, there is no
difference in taste or texture between thin and fat stalks (no matter
what you've heard!).
Cooking Asparagus: Since most of asparagus nutrients are in the
tip, its best to cook it upright in a tall container. Tie the
asparagus in a bunch after cleaning, place in standing position in 1-2
inches of boiling water. Cook just until stalks soften.
If you don't have a pan tall enough, try this: make a very light butter
sauce – I find that half butter and half virgin olive oil makes a
healthy and tasty sauce - in a wide, flat pan. Cool. Add a touch of
water and heat. Lay the washed wet asparagus in, cover and steam or
simmer for 3 to 4 minutes. I have a steamer that I lay the cleaned
asparagus in and steam, without butter or water, for about 5 minutes for
crunchy goodness. Sprinkle with seasoned rice vinegar or lemon for a
real treat!

Mich. Urges Locals to Eat Invasive Plants

EAST LANSING, Mich. (AP) - Area residents are being encouraged
to chow down on an invasive plant.
Garlic mustard is commonly tossed into landfills, killed with
herbicides or burned. But now it's being donated to food banks
to use in a variety of recipes provided by the Michigan Nature
Association.
The organization planned to hold a benefit dinner Saturday so
people can sample appetizers, soups, salads, entrees and even
desserts - all containing garlic mustard, the Lansing State
Journal reported.
Natalie Kent, development associate at the Michigan Nature
Association, came up with the idea for the "Eat the Invasive"
Garlic Mustard Project after spending a day pulling the plant
and throwing it away.
A co-worker mentioned that garlic mustard was edible, and Kent
was surprised.
"I thought, ... "why are we throwing this away if we can eat
it?'" Kent said. "This whole project evolved from that."
Garlic mustard is a threat to many of Michigan's native plant
species, but can be used in salads, bread, lasagna and added in
virtually any dish. The plant is found in many grassy areas and
even suburban back yards.
Garlic mustard used for the dinner and donated to food banks
comes from many of the 160 nature sanctuaries around the state
owned by the Williamston-based association. Member Brad
Slaughter is volunteering to pick the plant this month.
"This is really an innovative idea," Slaughter said. "It will
help make people aware of their surroundings."

2 Sides Do Battle in Court on Whether E.P.A. Should
Regulate Carbon Dioxide

WASHINGTON, April 8 - A federal appeals court heard arguments on Friday
in a five-year battle over whether the Environmental Protection Agency
has the authority to regulate carbon dioxide emissions from motor
vehicles.
In arguments on a suit that consolidates a number of legal actions,
opposing lawyers cited the same words of the Clean Air Act and drew
entirely different conclusions.
The plaintiffs are 12 states, a territory, 3 cities and 13 nongovernment
organizations, most of them environmental groups.
They have been seeking to have the E.P.A. explain why the Clean Air Act
does not empower the agency to regulate carbon dioxide emissions for
global climate considerations. That position of the Bush administration
is a reversal of Clinton administration policy.
Joining the government's side in the case are 11 states that oppose
carbon dioxide regulation and 19 industry groups, including those
representing car makers, refiners and chemical companies.
The arguments Friday, before a three-judge panel of the United States
Court of Appeals for the District of Columbia Circuit, reflected a major
fault line in current environmental battles: the extent to which carbon
dioxide can be linked to global warming.
The Bush administration has taken a skeptical view of global warming and
its effect. It has resisted international treaties, like Kyoto, that
govern some polluting countries and not others, and has worked to
protect industries that would be adversely affected if carbon dioxide
were regulated.
Exchanges between the judges and the lawyers focused on whether Congress
intended for the agency to regulate carbon dioxide even if its link to
global warming is uncertain and adverse effects on humans cannot be
accurately predicted.
In effect, the arguments came down to Congress's meaning when it wrote
into Section 202 of the act that the E.P.A. administrator "shall"
regulate any air pollutant from any new vehicles that "may reasonably be
anticipated to endanger public health or welfare."
"You don't have to look far to find the authority that the E.P.A. says
is missing," said the plaintiffs' lead lawyer, James R. Milkey, an
assistant attorney general of Massachusetts. Ignoring a broad reading of
the act, Mr. Milkey said, "is like saying a stop sign is not specific
enough."
But the section makes no specific mention of carbon dioxide as a
pollutant. Jeffrey Clark of the Justice Department argued that if
Congress had intended for the agency to regulate tailpipe emissions - a
quantum shift in policy that would have an enormous economic effect -
the lawmakers would have used language more comprehensive.
Judge A. Raymond Randolph, whose questions suggested that he did not
believe a clear link had been established between carbon dioxide and
global warming, cited another section of the Clean Air Act that says the
language referring to effects on public health includes "climate." Judge
Randolph told Mr. Clark, "I can't imagine anything turns" on a single
word.
"We agree," Mr. Clark responded.
Mr. Clark compared the current case to an effort by the Food and Drug
Administration under President Bill Clinton to regulate tobacco as a
drug. That effort was resolved when in 1997 the Supreme Court upheld the
tobacco industry's opposition, ruling that Congress had never intended
for tobacco to be considered a drug.
But Mr. Milkey called the comparison erroneous, declaring that if
tobacco came under F.D.A. jurisdiction, "it would be banned, and here
nobody is banning anything."
All three judges engaged in aggressive questioning of the lawyers,
although it was difficult to determine how the panel might ultimately
vote. While Judge Randolph seemed rough on Mr. Milkey, Judge David S.
Tatel was equally hard on Mr. Clark. The third judge, David B. Sentelle,
was skeptical of assertions from both sides, leading lawyers for the
environmental groups to wonder after the arguments if he could be the
swing vote.
Regulation of carbon dioxide emissions has been a major national
environmental issue since President Bush first won election. Despite a
campaign promise in 2000 to include carbon dioxide as an air pollutant
that should be regulated, the administration has resisted regulations on
such emissions from mobile as well as stationary sources. Conflict over
carbon dioxide is one major reason Congress has not passed new
antipollution legislation for power plants.
Motor vehicles account for about a quarter of the nation's carbon
dioxide emissions. Yet automakers have fought efforts to force them to
build more efficient engines, pointing to the extraordinary costs of
altering their factories to meet any new standards. The courtroom
audience Friday included lobbyists from leading car manufacturers.
The judges are expected to take as long as six months to issue an
opinion, after which an appeal is almost certain.
