For a Hospital Stay, Only a Select Few Qualify

The first time Mrs. Harris got a blood clot in her leg, back in 1998,
she spent almost three weeks in the hospital. So when the other leg
swelled up and began to throb last month, she came to the emergency room
with a good-size suitcase, ready to stay.
As it happens, she never even got to unpack. She was riding back home in
an ambulette barely 24 hours later with a sore leg, a bag of medicines
to dissolve her new clot, and a vague sense of unease.
Medicine is full of diagnostic subtleties. There are the small
symptomatic differences between the flu and pneumonia, the tiny visual
cues that differentiate malignant cells from normal ones, the almost
identical memory lapses of normal aging and senile dementia. But perhaps
the trickiest distinction of all these days is the one between an
inpatient disease and an outpatient one.
Hospitals in this country have become cruelly selective institutions of
higher healing. Only the most qualified applicants are admitted (and
then only a fraction of them get to unpack). The underlying financial
principles are simple enough: most admissions generate a lump sum
compensation. The simpler the illness, the smaller the sum. The longer
the stay, the further it must stretch. The hospital thrums to a simple
bottom line: get 'em in, get 'em out.
This credo does not translate particularly well into medical terms.
Criteria for hospital admission tend to follow an unwritten code
composed of local traditions, logical principles, idiosyncratic habits
and worst-case scenarios. Discharges are an elaborately negotiated
three-way truce among patient, doctor and illness.
It all sorts out in unpredictable ways. A life-threatening illness will
get you in - unless you are truly beyond all help, in which case you may
be encouraged to pass your last days at home. A mysterious illness may
get you in, but when the puzzle is solved you often convalesce
elsewhere. An illness requiring intensive nursing care may get you in -
or not, if devoted relatives will chip in at home. A famous name will
generally get you right in or right out, wherever it is you want to be.
Until a few years ago, a clot in the leg got you in and kept you there.
In view of the worst possible outcome - that the clot would move to the
lungs and cause respiratory collapse - observation and bed rest seemed
essential. To dissolve the clot, one blood thinner had to be given by
vein until a second oral one began to take effect, and levels of both
needed frequent monitoring, lest spontaneous bleeding occur.
Then the technology changed. The intravenous medication was reformulated
so that patients could inject it themselves, like insulin. The new drug
did not need such careful monitoring. Studies showed that bouncing
around at home with a clot in the leg was not particularly dangerous.
Frequent blood tests were still necessary, but patients could get them
as outpatients, and watch for bleeding themselves.
Lo, an outpatient disease was born.
Mrs. Harris, then, has a different disease than she did in 1998. Of
course, she has exactly the same problem, but now it is an outpatient
problem and thus somehow, by definition, imbued with less complexity,
less gravitas, less need for thought. The hospital staff got her in and
got her out.
Unlike her disease, though, not too much about Mrs. Harris herself has
changed since 1998. She still weighs close to 300 pounds, has a bad
heart and stiff lungs, and lives a good hour from the hospital. She is
still not very good at reading medication bottles or remembering drug
doses.
She still cannot stand giving herself injections. She still hates having
strangers (like visiting nurses) in her home. She is still the person
who began to bleed from some unidentified site in her intestine while
she was taking blood thinners the first time.
And so, it has been a hectic month for Mrs. Harris and her outpatient
caretakers, of whom I am one, what with the missed injections, confused
prescriptions, frantic phone calls, specially dispatched ambulettes and
blood tests that are consistently off the mark. So far, she is doing
fine despite it all. The rest of us are a little fatigued.
It is a cliché that once patients
remove their street clothes and slip on hospital johnnies, they become
invisible to medical staff, morphing from stockbroker, carpenter,
musician into just one more generic body to process.
Exactly the same holds true, of course, when they slip on a disease. All
their individuality tends to vanish behind its skimpy folds.
Would that someone had examined the lady with the blood clot in Room
9B24, discovered Mrs. Harris instead, overrode that inexorable get 'em
in, get 'em out, and kept her right there.

Review: Episiotomies May Bring More Risks

CHICAGO (AP) - For years, some doctors believed that an episiotomy, an
incision to enlarge the vaginal opening during childbirth, would prevent
spontaneous tearing that would be harder to repair. They also believed
the procedure would help women avoid incontinence and improve their sex
lives.
It turns out those beliefs were myths.
A new review of 26 research studies shows that episiotomies are linked
with a higher risk of injury, more trouble healing and more pain.
Episiotomies also had no effect on incontinence, pelvic floor strength
or sexual function. Women who had the procedure waited longer to resume
sex after childbirth. And their first post-birth intercourse caused them
more pain.
"This review puts together in one place all the evidence that we're not
getting the results we want," said Dr. Katherine Hartmann, the study's
lead author and a researcher at the University of North Carolina.
The review was published in Wednesday's Journal of the American Medical
Association.
An episiotomy is usually a small cut - deeper than the width of a large
metal paperclip and about as long, she said. But spontaneous tears often
are smaller and don't need stitches.
Many women and their doctors already know episiotomies haven't proven
beneficial, Hartmann said, adding that rates are dropping, but not fast
enough.
She estimated that 1 million women each year have unnecessary
episiotomies, citing studies that indicate they are done in about 30
percent of vaginal births.
An Associated Press analysis of hospital data found there were 616,702
episiotomies in 2002, but Hartmann said the procedures are
underestimated in hospital records.
Since 1983, the American College of Obstetricians and Gynecologists has
said episiotomies should not be done routinely. Dr. Laura Riley, of
ACOG's obstetrical practice committee, said most doctors already know
they should be done only when the baby is in distress.
"I think the message has been out there a while," she said. Her
hospital, Massachusetts General in Boston, only does them in about 8
percent of births, she said.
However, the numbers vary widely. Women are more likely to get an
episiotomy at some hospitals, Hartmann said. One study from the
mid-1990s found that episiotomy rates at Philadelphia hospitals varied
from 20 percent to 73 percent.
The Agency for Healthcare Quality and Research, which funded the review,
planned to post a summary on its Web site Tuesday afternoon. Pregnant
women should discuss it with their doctors or midwives, Hartmann said.
"Just in time for Mother's Day," Hartmann said. "This is a big gift for
future mothers."

A "dried unripe berry"
This week's fruit is a bit different and most
people have no clue that it's actually a fruit.
Known as Jamaican Pepper, Myrtle Pepper,
Pimento, Newspice, and Clove Pepper but most
commonly called Allspice! A "dried unripe berry" of the Pimenta dioica,
which belongs to the family Myrtaceae. 
The tree grows to a height of about 30 ft and has
aromatic, glossy green leaves and white flowers. The fruit berry, when dried,
is widely used as a spice. Its wood was once in such demand for the
making of walking sticks that the tree became endangered and was nearly
driven to extinction.
Spanish explorers happened on to the Allspice plant
in Jamaica in the beginning of the 16th century. The Spaniards thought
the fruits look like pepper so they gave it the name of Jamaican Pepper
and pimento from the word "pimienta", Spanish for peppercorn.
The English name "Allspice" was given because the
spice is said to have the aroma of several spices including cloves,
pepper and even cinnamon and nutmeg.
The green Allspice berries are picked and then
dried. They are very high in eugenol which is the main flavoring
substance of the berry.
Allspice is used to treat indigestion, flatulence,
and muscle pain. Allspice is an antioxidant, antifungal, and
anti-inflammatory to reduce pain and inflammation. It contains Vitamins
C, B-1, B-2 and beta carotene along with a few minerals. Allspice is
sometimes added to commercial medicines to improve their flavor.
Folklore has it that for centuries the Mayan
Indians used Allspice to embalm the bodies of important leaders.
Allspice is available as whole berries or ground.
Grinding your own whole berries will provide the most intense flavor and
longest shelf life.
Allspice is used in seasonings, sauces, sausages,
ketchup, jams, pumpkin, gravies, roasts, hams, baked goods, and teas.
Caribbean cooking relies on Allspice as the main ingredient in jerk
seasoning. It is used in Caribbean, Mexican, Indian, English, and North
American cooking and in seasoning blends such as jerk seasoning and
curry. Allspice can be used in just about any food you can think of
adding nutrients, flavor and taste.

Prostate Cancer Survival Rates Studied

CHICAGO (AP) - Men with nonaggressive prostate cancer who were
treated with hormones or took no action at all are unlikely to
die of the disease even 20 years later, new research shows.
Conversely, the disease is likely to be deadly for men with
signs of more aggressive cancer treated with hormones or just
observation.
The new study, with an average follow-up of 24 years, suggests
men with low-grade, localized cancers might do well with little
or no treatment, avoiding the surgery or radiation that can
cause impotence and incontinence.
The findings are the latest to support a cautious approach in
treating some prostate cancers, although there is still debate
about how to define the riskiest cases. Prostate cancer is the
most common major cancer, and a man's lifetime odds of getting
it are 1 in 6.
The research, led by Dr. Peter Albertsen at the University of
Connecticut, appears in Wednesday's Journal of the American
Medical Association. It involves 767 Connecticut men aged 55 to
74 who were diagnosed between 1971-84 with cancer that had not
spread beyond the prostate gland and who received either hormone
therapy or observation - sometimes called "watchful waiting."
Prostate cancer ended up killing 228 men, mostly within the
first 15 years after diagnosis; most of those men had high-grade
tumors - those with cells that looked very abnormal under a
microscope.
The study contrasts with a smaller, Swedish study published in
JAMA last year suggesting that untreated, slow-growing localized
tumors could turn deadly after 15 years.
The new study casts doubt on the earlier findings and suggests
that "if you survive 15 years, it is unlikely that the tumor
will turn ugly and progress," Albertsen said.
However, Dr. Peter Gann of Northwestern University, who co-wrote
a JAMA editorial, cautioned that the studies aren't directly
comparable because the Swedish men might have had different
types of tumors and received hormone treatment later.
Gann said the new study reinforces the importance of measuring
tumor grade in predicting outcomes for men with localized
disease that has not spread. In the study, only 7 percent of the
men with low-grade tumors died of prostate cancer during the
study, compared with 66 percent of men with high-grade.

LENA'S COMMENT: This really isn't new news as I read
almost the same article many years ago in another JAMA! This
medical journal goes to every doctor that holds a license yet
they will continue to put eighty percent of men through
unnecessary hell with their cut or poison attitude. Yes
80% of prostate cancer isn't a fast growing deadly
prostate cancer... Do you know why doctor's continue to give
people false information? Because they do not read their
journals!!! I've filed many away and was the only person in the
office who read them... Their attitude of cancer is treat it
whether it's proven safe and effective or not. "We can at
least give them hope!" is the most often statement out of a
doctor's mouth that I heard when I questioned them why do this
when not necessary. I know I heard it so very many times over my
20+ years in medical offices... It doesn't matter that
they are actually lying to them with that attitude... It
behooves me to understand how they can continue that lazy and
life threatening attitude, but you do not have to listen nor do
as they suggest!!!

When Fruit Flies Send a Messy Message

In the home, the fruit fly can be a telltale indicator of neglect. When
Drosophila melanogaster starts flitting around your fruit bowl, you know
there's a forgotten peach in there somewhere.
On a much larger scale, the tiny fly can also be a useful marker for
environmental neglect, Australian researchers report.
They've shown that over two decades in eastern Australia, fruit flies
have undergone a genetic change as the climate has warmed.
"Fruit flies are model organisms for studying all sorts of things
genetically," said Dr. Ary Hoffmann, a professor at the University of
Melbourne and an author of a report on the finding published last week
in the journal Science. "What you've got here is a situation where in
nature over a short period of time, they have been adapting to climate
change."
Dr. Hoffmann and his colleagues focused on one gene related to
metabolism. This gene has two forms, or alleles - one that is adapted to
hotter, tropical conditions, the other to cooler conditions.
Studying the genetic makeup of fruit flies along the east coast and
comparing the data to similar analyses from 1979 and 1982, the
researchers saw the tropical allele starting to show up in areas where
the cold allele had been predominant.
Average temperatures in eastern Australia have increased by about 1
degree Fahrenheit over that period. This being the Southern Hemisphere,
the colder allele had been more common in the southern parts of the
coast, and the tropical allele more common toward the north, closer to
the Equator.
"The genetic constitution of the population is shifting so it's more
like the form seen in the north," Dr. Hoffmann said.
It's likely that other organisms will show similar genetic changes, Dr.
Hoffmann added, so they may prove useful as markers for climate change
in other regions. "What we need to start looking at is any single
widespread species that covers a range and see if it shows this kind of
change," he said.
But it would also be interesting to look for climate-related genetic
changes in species that have a very restricted range - those that are
stuck in a rain forest, for example. "In that case, they can't move,"
Dr. Hoffmann said. "But there may be rapid adaptation at a basic level."
