Free flu vaccine fails to reduce cases

study: 'Rates haven't decreased and there's been lots of money spent'

Canada's first experiment in universal, free flu vaccine has cost Ontario taxpayers more than $200-million, but appears to have done nothing to cut the spread of influenza, a new study suggests.

Per-capita flu rates in the province have not fallen at all since the
program was introduced in 2000, concluded the University of Ottawa research, published in the journal Vaccine.

In fact, the average monthly incidence of the virus jumped over the first
five years of the program, though researchers say it is too early to say that numbers are really on the rise.

"All we do know is rates haven't decreased, and there has been a lot of money spent," said Dianne Groll, the University of Ottawa professor who led the study. "The program was designed to reduce the incidence of flu, and this hasn't yet happened."

The heavily publicized offer of free flu shots to all 12 million Ontarians was launched in 2000 to try to ease pressure on emergency wards from flu patients, and cut the incidence and severity of the illness.

Dr. Groll said it might be wise to focus on better targeting the vaccine at high-risk groups -- the very young, the elderly and the chronically ill -- perhaps by giving shots to anyone who comes into an emergency ward.

She also said the province failed to collect detailed "baseline" information on vaccination patterns before the plan started or similar data since then to compare, making it difficult to thoroughly evaluate the program.

Some infectious disease specialists warned yesterday the study is seriously limited, partly because it compares the rates of laboratory-diagnosed flu before and after the program started. Cases that are confirmed by a lab test represent a small fraction of the total amount of flu and may not give a true picture of the situation, said Dr. Allison McGeer, one of the country's leading influenza experts.

The Ottawa research correctly and interestingly analyzed the data available but is "irrelevant," concluded Dr. McGeer, infection-control officer at Toronto's Mount Sinai Hospital and a strong proponent of universal immunization. She acknowledged there is simply very little evidence with which to evaluate the program.

"That we would publish this study is a marker of desperation."

Dr. McGeer said there is other research, not yet published, suggesting the campaign has resulted in more chronically ill seniors being vaccinated and has reduced the seriousness of some illness.

The Ontario initiative, the world's first universal flu campaign, distributes about five million vaccine doses a year at a current cost of $50- to $55-million, including promotion, according to the Health Ministry.

Dr. Groll looked at the number of cases of laboratory-diagnosed flu reported to Health Canada between 1990 and 2005.

She found that the rate between 1990 and 2000, when the program started, was 109 per 100,000 Ontarians. After the campaign's launch, it jumped to 164.

The data was analyzed by Queen's University statistician David Thomson, who concluded there are not enough statistics yet to prove the numbers are on the rise.

The researchers also found the Ontario flu rates did not change relative to other provinces. And, though the number of laboratory tests conducted has gone up, it has not increased compared with the rest of Canada.

Dr. Michael Gardam, infection-control specialist at Toronto General Hospital, also stressed the limitations of the study, noting the vaccine in one of the years after the program started did not match the strains circulating in Ontario.

Still, he admitted that emergency rooms at his hospital are no less busy and that "Ontario still gets flu outbreaks like anybody else."

"The most we can say from this study is that clearly this program is not a home-run success," he said. "If there is success, it's subtle. If the results are that subtle, should the money be spent elsewhere? That is a very good question."

Dr. Groll noted the campaign was instituted in 2000 without any "baseline" data -- information on what sort of people were getting vaccinated before,
how many of those got the flu and whether flu patients used hospital emergency wards.

Nor has such information been gathered since, making it very difficult to effectively evaluate the global vaccination plan, she said.

"The program just lacks the baseline information you would need -- that anybody would need -- before anybody put this kind of ulti-million-dollar proposal together," Dr. Groll said.

John Letherby, a spokesman for the Ontario Health Ministry, defended the flu program, though, saying it does have many "ancillary benefits" apart from the impact on overall rates.

Those benefits include less emergency-room crowding and less absenteeism, but Mr. Letherby said the ministry does not itself collect such statistics.

As reported in Pediatrics, researchers at Winthrop University Hospital in Mineola, N.Y., surveyed 161 physicians about how they treat fevers. Most doctors said they start treatment when a young patient's temperature reaches 101 degrees. And half said they alternated between doses of acetaminophen (Tylenol) and ibuprofen (Advil or Nuprin). When the doctors were asked why they switched between those two drugs, almost a third said they were following a recommendation by the American Academy of Pediatrics. But according to the researchers, the American Academy of Pediatrics never issued any such advisory. In fact, the study's authors say, there have been no definitive studies on the best way to treat a fever -- in either children or adults.

People, Pets Sickened by Boot Spray

ATLANTA (AP) - A waterproofing boot spray has sickened nearly 200 people and more than two dozen pets in the Midwest since early last year, health officials said Thursday.

No one died, but people in five states reported coughing and breathing problems, and more than 80 went to hospitals. A 69-year-old Michigan man was put on a ventilator and hospitalized for 19 days, said Susan Smolinske of the regional poison control center at Children's Hospital of Michigan in Detroit.

Three housecats died after exposure to the aerosol products, Smolinske said.

Most of the cases were linked to two Canadian products - Jobsite Heavy Duty Bootmate and Rocky Boot Weather and Stain Protector. Both are manufactured by Assured Packaging of Mississauga, Ontario.

The U.S. Centers for Disease Control and Prevention detailed the investigation this week in its Morbidity and Mortality Weekly Report.

"We're absolutely sure it's the products that caused these illnesses," said Smolinske, lead author of the MMWR article.

Company officials did not immediately respond to an Associated Press request for comment. When a reporter offered to forward the CDC report, an employee declined to provide e-mail addresses.

In more than half the cases, the products were sprayed indoors. But shoes and boots sprayed in garages or outdoors still caused illness when they were brought indoors as the product evaporated, investigators found.

The boot sprays were pulled from all U.S. store shelves in January, after the Michigan Department of Community Health asked a product distributor to issue a recall. However, some people may still have the products in their homes and more may become ill, Smolinske said.

The Consumer Product Safety Commission is looking into the boot sealant products and the illness reports. Health officials are continuing to test the product and investigate what chemical might have caused the illnesses.

Health officials have reports of illnesses in 185 humans and 29 animals. Most of the cases were in Michigan, with others in Indiana, Pennsylvania, Ohio and Kentucky.

The cases were reported to hospital-based poison control centers. The first reports were in February 2005 but cases still are coming in, Smolinske said. The Assured Packaging products were first marketed in 2004, she said.

All but 23 of the case reports were tied to the Assured Packaging. In 21, the consumer couldn't recall what product they had used, and two cases were linked to a product by another manufacturer, she said.

Apple Of The Orient aka Ebenaceae, Diospyros kaki

In a few short weeks as we make our way to Southern California for my grand daughter's graduation I look forward to being able to pick this fruit from a tree? Not one I always ate or knew about but several years ago we arrived home from the grocery store one afternoon to find these squatty orange persimmons sitting at our back door. I put them in the kitchen as they were really hard and persimmons are not one of my favorite foods so I would not hurry to eat one. Later in the evening I received a call from a dear friend asking me if I had found the fruit at our back door. I assured him I did. He wanted to know if we had eaten them and my reply was. "Are you nuts those persimmons are hard as a rock and not ready to eat!" He started laughing and said, "Oh no go get one and a knife and slice it as you would an apple and I'm not getting off the phone until you do!" In order to clear my phone it looked like I would have to do his bidding? When sliced into it appears I saw this beautiful unique star fish pattern inside. So, I grudgingly did as he said while. I bit into a slice and waited for disappointment. If you've ever bitten into a native persimmon or an unripe Japanese variety, you know that they can be puckery bitter when not fully ripe. My taste buds received a huge shock as the taste was not at all the flavor of a persimmon. A new favorite food had entered my life that day. I now wait patiently every year until I can get some.

That favorite food is a Fuju Persimmon. And some things I have learned about it is very interesting is that it's;

A short squatty orange fruit known as the Apple of the Orient (Ebenaceae, Diospyros kaki). Fuju looks like a bright orange, medium-sized squatty tomato, is perhaps best-known in America as the Japanese, or Oriental, persimmon, occasionally Chinese plum or, when dried, Chinese fig. In Spanish, it is called caqui. Native to Japan, Burma, northern India and China, the tree can be found growing at altitudes from 6000-8000 ft (1830-2500m) Marco Polo wrote about the persimmon trade in the early 14^th Century, and it is featured in ceremonies throughout Asia, where over two thousand cultivars exist. It was not until the late1800?s that grafted trees were distributed to California, but by 1930, there were close to 100,000 bearing trees around the state. The trees do well in the mild climate portions of California, and over 90% of the total U.S. persimmon fruit crop is grown there.

Eaten like an apple - has a crisp texture that makes it good for slicing and using of fruit trays or in fruit salads. Persimmons are used in main dishes to enhance the flavors in puddings, desserts, salsas, jams, candy, breads, cakes, stewed fruits, and salads or they can be eaten fresh, peeling and all. Dried persimmons, which have a chewy, jelly-like texture and unique sweet flavor, are also available.

Rich in nutrients, particularly such important vitamins as A and C, a little of the Bs, and E. Many minerals, such as calcium, magnesium, copper, and manganese, a little iron, and other trace minerals, are also present, when they are contained in the water and soil that nourishes the plants or trees. Fuju's are low in fat and high in fiber making for a great bowel stimulator.

Do You Seriously Want Healing and/or Health?

Dr. Garland works at helping people overcome their illness and disease, of which cancer seems the most ominous. Monsanto Continues French GMO Tests Despite Ruling

PARIS - The French arm of US biotech giant Monsanto said on Friday it would continue experiments on genetically modified (GMO) crops in France despite a court ruling stopping some of its tests.

France's top court, the Council of State, last week cancelled two authorisations granted to Monsanto to grow test fields of its genetically modified Roundup Ready maize.
The court said Monsanto had not given precise enough details of where the field trials would take place.

The annulment was welcomed by anti-GMO campaigners who said it showed experiments were held in secret and repeated calls for all trials in the country be halted.

But Monsanto, an industry leader in genetically altering crops, said it would not prevent them to carry on their tests.

"We'll continue our experiments but we might have to change a bit the way we ask for licences in the future," Yann Fichet, Monsanto France's director of external relations told Reuters.

The two cancelled licences were granted in 2004 to grow a certain type of GMO maize resistant to the Roundup insecticide for several years. One of the two licences was out-of-date. The other could have been used this year, Monsanto said.

The company, which submitted four additional testing requests this season concerning 17 sites and is waiting for an approval from the French farm ministry in the coming days, said it had always provided all details concerning its experiments.

"Every year we provide the farm ministry an exact list and detailed maps of the experiments we intend to do," Fichet said.

FRENCH TESTING RECONSIDERED

Fichet nevertheless admitted that for licences lasting several years -- as it was the case for the licences cancelled -- the company does not hand the list of locations every year to the French genetic engineering commission, as the Council of State says it is requested under the French law.

"We don't want to do so because then our experiments are destroyed by the (GMO) opponents," he said.

Opposition to GMO crops has been virulent for years in France where a large majority of the population says it will refuse to eat GM foods as long as it is not proven they are safe for human consumption in the long run.

Yet, France applies EU legislation that permits some GMO maize to be grown and experiments on several types of GM crops to be carried on under precise rules and after approbation.

This infuriates opponents including those who regularly destroy field experiments, claiming the tests lead to irreversible contamination of conventional plants.

Half of Monsanto's experiments in France are destroyed each year, Fichet said.

Last week France's maize growers group AGPM said many farmers who this year had planted up to 5,000 hectares of GMO crops for commercial sale, 10 times the area sown in 2005, had asked to remain anonymous to avoid attacks on their crops.

"Transparency today is used to destroy our work. This is unacceptable," Fichet said. "It certainly makes us think deeply about our experiments in France."
