Inhaled
corticosteroids (ICS) solves one problem while raising the risk of
weakened bones.
Now a new study demonstrates how yet another dangerous risk may be
created for those who treat asthma, chronic obstructive pulmonary
disease (COPD), allergies, arthritis and other conditions with high
doses of ICS. Individual case reports indicate that ICS use may trigger
atrial fibrillation, the most common type of abnormal heart rhythm, or
arrhythmia. Atrial fibrillation sharply increases the risk of heart
failure and stroke.
To explore the association between ICS use and atrial fibrillation,
researchers at Erasmus University in the Netherlands analyzed data
collected from nearly 8,000 adults over the age of 55. During the
nine-year study period, 385 subjects were diagnosed with new-onset
atrial fibrilation, When this subgroup was compared to the others,
researchers found a clear link between atrial fibrillation and high-dose
use of ICS. No link was found in patients who used low or intermediate
doses.
In the current issue of the Archives of Internal Medicine, the authors
note that the high levels of corticosteroids could set the stage for
arrhythmia by throwing off the balance of potassium and sodium. ICS may
cause sodium retention, setting up AF risk factors such as congestive
heart failure and high blood pressure.
A Harvard study has already shown how the regular use of inhaled
steroids causes bone loss at the hip in women between the ages of 18 and
45. The more "puffs" taken each day, the greater the rate of bone loss.
And while the study specifically focused on premenopausal women, it
noted that continued ICS use after menopause could contribute to even
more bone deterioration and greater risk of fracture.
But men don't escape this risk. In a 2004 study that reviewed medical
records for more than 40,000 subjects (almost all males) diagnosed with
COPD, researchers identified about 1,700 cases of COPD patients with
nonvertebral fractures, and matched them with more than 6,800 control
subjects.
After a one-year follow-up, general ICS use was not associated with a
higher fracture risk compared to patients who didn't use inhalers. But
fracture risk was found to be significantly elevated among those who
used the highest dosages of ICS.

US lawmakers question FDA OK of
Sanofi antibiotic

WASHINGTON (Reuters) - Several U.S. lawmakers on Monday
questioned the U.S. Food and Drug Administration's 2004 approval
of Sanofi-Aventis SA's antibiotic Ketek, raising concerns about
faulty data and asking for internal agency documents.
At issue is whether the FDA dismissed problems with a major
trial after several physicians were cited by agency
investigators for falsifying data or improper record-keeping,
said the lawmakers, two Democrats and a Republican.
Ketek, approved to treat respiratory infections, drew renewed
scrutiny in January when researchers reported that three
patients using the drug developed severe liver damage and one
died.
Senate Finance Committee Chairman Charles Grassley said he was
troubled that the FDA approved Ketek despite unresolved
questions about the drug's safety and effectiveness.
In an April 27 letter to the agency, released on Monday, the
Iowa Republican said some information supporting its approval
was "beset by systemic data integrity problems."
Democratic Reps. Edward Markey of Massachusetts and Henry Waxman
of California, in a letter sent Monday, also questioned the
Ketek approval process.
The FDA had rejected the drug in 2001 and 2003, asking for more
safety information. After the first rejection, the company began
the trial in question, known as 3014.
Officials for the private contractor that oversaw the trial
notified the Paris-based drugmaker about the 3014 data problems,
according to a report in Monday's Wall Street Journal.
FDA investigators later discovered discrepancies, the newspaper
said. In one case, an Alabama doctor pleaded guilty fraud and
was sentenced to jail.

Disputed Pesticide to Stay
in Stores
Dichlorvos is a known carcinogen in California but
the federal EPA will allow it to be sold under new restrictions.

WASHINGTON — The Environmental Protection Agency has tentatively agreed
to new restrictions that will allow a Southern California pesticide
maker to keep a controversial insecticide on the market, the agency
announced Tuesday.
Newport Beach-based Amvac volunteered to cancel some uses and add
restrictions to others for a pesticide known as dichlorvos, or DDVP,
which is commonly used to kill mosquitoes, fleas and other insects, the
EPA said.
Amvac acted after EPA officials provided the firm with the results of an
ongoing health assessment that raised questions about risks associated
with exposure to the chemical, which is related to World War II-era
nerve agents.
DDVP is listed by California as a known carcinogen and is part of a
class of chemicals that has been linked to developmental damage in
children. Exposure to DDVP can cause flu-like symptoms, including
headaches, nausea and vomiting. In large doses, the chemical is fatal.
"If these changes [in usage] do occur, these products can be used
safely," said James Jones, director of the EPA's office of pesticide
programs.
In a statement, Amvac said it did "not necessarily agree" with all
details of the EPA's risk assessment, but said the agency had "employed
sound science in reaching its major conclusions."
"As Amvac has always based its policy and regulatory decision making on
sound science, it felt that this mitigation was acceptable," the
statement said.
Environmental groups, which consider DDVP one of the most dangerous
pesticides on the market, said that the proposed restrictions do not go
far enough. They noted that the EPA previously considered banning most
residential uses of DDVP, and they accused agency officials of "illegal"
meetings with Amvac that did not allow adequate public participation.
"All of these uses inside the home are dangerous things. That's what is
of most concern," said Aaron Colangelo, an attorney with the Natural
Resources Defense Council. "They're allowing them to stay on the
market."
DDVP is used in numerous products, including pet collars, pest strips
and aerosol sprays. It is sold under a number of names, including Alco
No-Pest Strip, Amvac Insect Strip and Swat Pest Strip.
The chemical has both residential and commercial uses, mostly in niche
areas such as the fumigation of storage areas for peanuts and
pistachios, and mushroom farms.
Under Amvac's proposal, consumers will continue to be able to purchase
pest strips, which release minute quantities of DDVP for months, killing
insects by attacking the nervous system.
The size of some strips will be reduced. Labels will be modified to warn
that larger strips should not be used in homes, except in garages,
attics and crawl spaces occupied for less than four hours per day.
Smaller strips will be limited for use in closets and cupboards, so long
as children or the elderly are not present.
In addition, Amvac will cancel DDVP products used as home foggers, on
the lawn, and for treating cracks and crevices.
The EPA said that after a period for public comment, it would finalize
the proposal in the coming weeks. The products would then be allowed to
remain on store shelves for 18 more months. Jones said the EPA did not
believe there was an immediate threat from using the products.
"We don't think people will be exposed in a way that will cause harm,"
Jones said.
DDVP has a long and contentious regulatory history. The EPA raised
concerns about it as far back as 1982, and it has been undergoing a
special safety review since 1988.
Amvac officials have fiercely defended their product, saying that DDVP
caused no long-term damage to humans and that the product plays an
important public health role in killing disease-carrying pests.
The company was at the center of a controversy in the late 1990s, when
it proposed demonstrating the safety of its product by testing human,
rather than animal subjects. Environmental groups raised ethical
concerns over the tests.
After a drawn-out court battle, the EPA finalized a rule this year
allowing limited consideration of human test data. Last month, the EPA's
Human Studies Review Board delivered a mixed verdict for DDVP, saying
that the tests allowed the EPA to loosen safety standards for short- and
medium-term exposure, but not for chronic exposure possible with pest
strips, which remain potent for up to four months.
