Public Citizen Seeks Cholesterol Drug Ban

WASHINGTON (AP) - A consumer advocate is urging the government to ban
the new anti-cholesterol drug Crestor, citing new cases of a
life-threatening muscle side effect among patients taking a low dose
thought to minimize the risk.
Public Citizen's renewed call comes days after Crestor's maker,
AstraZeneca, wrote doctors in Britain urging them to start patients on a
mere 10-milligram dose because of concern about that muscle-destroying
condition, rhabdomyolysis.
Food and Drug Administration records show 11 cases of the muscle
condition reported to the agency since late February, on top of 14
previously known - and seven of those newest patients were taking the
10-mg dose, said Sidney Wolfe of Public Citizen.
Nine of the new cases required hospitalization; at least five were under
age 50, he said.
Crestor "is a doomed drug," Wolfe wrote the FDA. "We renew our effort to
get the FDA to ban this uniquely dangerous drug before it does any
further damage."
FDA spokesman Brad Stone said the agency is closely monitoring Crestor.
He said, however, that many of the side-effect cases reported so far are
among patients with other risks for the muscle problem.
"So far, what we've seen doesn't indicate a disturbing pattern, but
we'll continue to look at the data" from Wolfe and other sources, Stone
said Monday.
A spokesman for AstraZeneca said the drug's safety "is totally
consistent" with competitors in the popular cholesterol-lowering statin
family.
Crestor was approved last August, although seven cases of rhabdomyolysis
had occurred in patients taking an 80-mg dose during studies.
Rhabdomyolysis is caused by a breakdown in muscle fibers that end up
circulated in the body. Those fibers can be toxic for kidneys and can
cause them to fail.
The condition, characterized by muscle pain and weakness, is very rare.
It prompted another statin, Baycol, to be pulled off the market in 2001
after dozens of deaths worldwide.
But FDA ultimately declared Crestor an important option for some
patients because it appeared slightly more potent than other statins. To
lower the side-effect risk, the agency recommended starting doses of 5
mg to 10 mg, and warned never to exceed 40 mg.
Wolfe initially urged a Crestor ban in March, citing seven more
rhabdomyolysis cases in the drug's first months of sales, including a
39-year-old American who died after taking a 20-mg dose.

Medtronic Recalls Insulin Infusion Sets
More than 160,000 insulin infusion sets are being recalled because of
problems that the California manufacturer says may have led 200
diabetics to be hospitalized for elevated blood-sugar levels.
Diabetics who use the Paradigm Quick-set Plus brand should contact
Medtronic MiniMed immediately to exchange any unused infusion set for a
free replacement, the company said Tuesday. Call 1-800-MINIMED, or check
the Web site at .http://www.minimed.com/QSP
The recall applies to all Quick-set Plus models, a newer version of an
infusion set that Medtronic has discontinued. Its older brands are not
affected by the recall, and will be used as replacements.
The infusion set delivers insulin from an infusion pump to a patient's
body.

infection to a head. Also beneficial in the removal of dead skin tissues
as for cosmetic use in facial scrubs, and in a skin disease involving
lack of pigmentation in patches of skin (vitiligo).
Both fresh and dried figs are great medicinals! Dried, has about 5 times
the amount of nutrients as fresh.
The fig is very rich in fiber and a host of serenity producing vitamins
B1, B2, B5 and B6, along with heart healthy magnesium and Vitamin E.
Bone and muscle builders Calcium and Iron reside in the fig as well.
Stressed out? Eat figs!
Figs are a good source of potassium, a mineral that helps to control
blood pressure. Since many people not only do not eat enough fruits and
vegetables, but also consume high amounts of sodium as salt is
frequently added to processed foods, they may be deficient in potassium.
Low intake of potassium-rich foods, especially when coupled with a high
intake of sodium, can lead to hypertension. In the Dietary Approaches to
Stop Hypertension (DASH) study, one group ate servings of fruits and
vegetables in place of snacks and sweets, and also ate low-fat dairy
food. This diet delivered more potassium, magnesium and calcium. Another
group ate a "usual" diet low in fruits and vegetables with a fat content
like that found in the average American Diet. After eight weeks, the
group that ate the enhanced diet lowered their blood pressure by an
average of 5.5 points (systolic) over 3.0 points (diastolic).
Great for intestinal balance and cleansing Figs have the highest dietary
fiber of any common fruit, nut or vegetable. They are high in peptic
fiber, which is a soft soluble fiber and helps the body remove toxic
wastes and reduces cholesterol levels. They are good to prevent and
relieve constipation. Chinese medicine recognizes figs as a toxin
neutralizer. One of the most alkalizing foods, figs can help balance the
acid that forms from diets rich in meat and refined foods. Figs contain
significant amounts of the proteolytic enzyme Ficin which is a digestion
enzyme helping with the digestion especially of proteins.
Fig sugar content and glycemic index is very high. Therefore it is
preferable and smarter to eat them with a meal or as a dessert rather
than on their own as a snack.
Leaf Has Insulin-Lowering Properties
You probably do not think about the leaves of the fig tree as one of
fig's edible parts. But in some cultures like Greece, stuffed fig leaves
or dolmas are a common part of the menu, and for good reason. The leaves
of the fig have repeatedly been shown to have antidiabetic properties
and can actually reduce the amount of insulin needed by persons with
diabetes who require insulin injections. In one study, a liquid extract
made from fig leaves was simply added to the breakfast of
insulin-dependent diabetic subjects in order to produce this
insulin-lowering effect.
Word of caution; check that the figs you are buying are organic
and of good quality. Fumigation is sometimes used to assure the absence
of insects in figs and sulphur dioxide is used to maintain their sweet
flavor and rich color. Dried figs infested by mites may cause severe
gastrointestinal conditions if eaten. Mold attack may cause figs to form
mycotoxins.

Study: Medicare Favors Private Insurers

WASHINGTON (AP) - Recent changes in Medicare are giving private
insurers a bigger advantage than previously thought, says a
study by a private foundation.
The Commonwealth Fund, which advocates research on health
issues, said in the study released Thursday that the government
is paying managed care plans 8.4 percent more on average than
costs in traditional fee-for-service Medicare.
The difference amounts to an average of $552 more for each of
the 5 million Medicare beneficiaries enrolled in managed care
plans, or a total of $2.75 billion in 2004, the report said.
The study by the New York-based foundation updates earlier work
by a government advisory panel that put the difference at 7
percent.
The higher payments result from the Medicare law that President
Bush signed in December. The law gives private insurers a
broader role in the government health care program for older and
disabled Americans.
The extra payments - 17 percent over two years - are aimed at
increasing enrollment in health maintenance organizations and
other managed care plans, which supporters say will help lower
Medicare costs over the long run.
Provisions of the new law to benefit those plans, however,
"explicitly increased Medicare costs in 2004 and through 2013,"
the report said.
Government estimates estimated the extra payments at $14 billion
to $46 billion over 10 years.
HMOs are using about half the money to cut premiums and expand
services; much of the remainder goes to increased payments to
health care providers. The companies are keeping about 5 percent
of the increase, the report said.
America's Health Insurance Plans, the lobbying arm of the
managed care industry, criticized the report for overstating the
difference in spending.
"It's a head-in-the-sand kind of analysis," said Karen Ignagni,
the president and CEO of the managed care association. "The
underlying goal here is to go back to no choices for
beneficiaries."
Plans known as Medicare Advantage plans generally provide a
wider range of benefits but limit patients' choices of doctors
and other medical services. In the Medicare fee-for-service
program, clients choose their health care providers.
The report assumed that people enrolling in managed care plans
are no more sick or well than seniors in traditional Medicare.
Government studies, though, have found that healthier seniors
tend to sign up for managed care.
If the report had factored in the health of managed care
enrollees, Medicare would be paying even more on average because
the managed care plans receive fixed amounts, regardless of
whether seniors use medical services, said Brian Biles, a George
Washington University professor and an author of the report.
The health insurance lobby has said the higher government
payments are a step toward stabilizing managed care plans under
Medicare. Skimpy Medicare reimbursements, which averaged 2
percent in 2003 to Medicare HMOs, had been driving insurers from
the market.
Opponents of the law contend the increases are set up to lure
the elderly to managed care plans as part of an effort to
undermine traditional Medicare.
Air Pollution Linked to Genetic Mutations

WASHINGTON (AP) - Sooty air pollution can cause genetic damage that can
be passed along to offspring, Canadian researchers reported Thursday in
a study on mice.
Follow-up work is needed to learn if people can inherit
pollution-damaged DNA that harms their health. In the meantime, the
discovery is sure to increase scientists' worry about particulates, the
microscopic soot particles emitted by factories, power plants and
diesel-burning vehicles.
The good news: Air filters protected the mice.
"The new work now adds another area of potential concern" because of the
implications for risks to future generations, said Dr. Jonathan Samet of
Johns Hopkins University, who headed a recent National Academy of
Sciences call for more research into the dangers of this common
pollutant.
These airborne particles are linked to a growing list of health
problems, including asthma and heart disease, in the people who breathe
high levels of them.
But there had been little evidence that any air pollutant might cause
the kind of genetic damage that can be inherited - until Canadian
scientists in 2002 housed mice downwind from steel mills and tested
their offspring. The males passed on double the DNA mutations as mice
living in the cleaner countryside.
The question was why.
Now those same researchers, from Ontario's McMaster University, report
in the journal Science that they've found the culprit: airborne
particulate matter, better known as soot.
In the new experiments, biologist James Quinn and colleagues housed two
groups of mice near the steel mills for 10 weeks. One group breathed
outside air, while the other was housed in a chamber equipped with HEPA
filters - high-efficiency air filters designed to catch microscopic
particles.
Then, the mice were bred and their offspring checked for specific DNA
mutations that are passed through the father's sperm.
Mice that breathed filtered air had mutation rates 52 percent lower than
the mice exposed to full-strength steel mill pollution.
The specific sperm changes measured aren't linked to disease, but
they're similar to a type of DNA damage that is. Quinn said more study
is needed to see if they're a marker for potential health problems, and
whether pollution-spurred mutations in disease-causing genes could be
inherited, too.
Regardless, Quinn said the study's practical value may be in showing the
effectiveness of air filtration. The HEPA filters blocked particulates,
and nature does the same thing - particulates adhere to tree leaves -
which has implications for policy-makers who must decide on
road-building and tree-cutting projects, he said.
Quinn couldn't say if the particulates themselves or toxic chemicals
that attach to them damaged the sperm. But one suspect is a group of
particulate-clinging chemicals called polycyclic aromatic hydrocarbons,
or PAHs, some of which are known to be cancer-causing.
Air samples showed daily PAH exposure near the steel mills was 33 times
as high as in the nearby cleaner countryside - but HEPA filtering of the
urban air blocked most of those chemicals as well, the study concluded.
The Environmental Protection Agency already has ordered tougher curbs on
ultra-fine particulate pollution because of concern about effects on the
elderly, children and people with respiratory illnesses. In December, it
plans to reveal which parts of the country aren't in compliance.
Tiny enough to be inhaled deeply into the lungs, these particulates
enter the bloodstream and move through the body.
"At the moment, we are grappling with the fact that even though the air
is visibly cleaner, we're still finding adverse health effects" from
particulates, said Hopkins' Samet.
If those same particles make it all the way to sperm-forming cells,
"that would be quite a remarkable sequence" - one that needs
confirmation, Samet cautioned. Still, the potential for affecting next
generations makes it "both a public health issue and an issue for the
ecosystem," he said.
