Vitamin K at Birth: To Inject or Not

Newborn infants routinely receive a vitamin K shot after birth in order
to prevent (or slow) a rare problem of bleeding into the brain weeks
after birth. Vitamin K promotes blood clotting. The fetus has low levels
of vitamin K as well as other factors needed in clotting. The body
maintains these levels very precisely. Supplementation of vitamin K to
the pregnant mother does not change the K status of the fetus,
confirming the importance of its specific levels.
Toward the end of gestation, the fetus begins developing some of the
other clotting factors, developing two key factors just before term
birth. It has recently been shown that this tight regulation of vitamin
K levels helps control the rate of rapid cell division during fetal
development. Apparently, high levels of vitamin K can allow cell
division to get out of hand, leading to cancer.
What's the Concern?
The problem of bleeding into the brain occurs mainly from three to seven
weeks after birth in just over five out of 100,000 births (without
vitamin K injections); 90 percent of those cases are breastfed infants
because formulas are supplemented with unnaturally high levels of
vitamin K. Forty percent of these infants suffer permanent brain damage
or death.
The cause of this bleeding trauma is generally liver disease that has
not been detected until the bleeding occurs. Several liver problems can
reduce the liver's ability to make blood-clotting factors out of vitamin
K; therefore extra K helps this situation. Infants exposed to drugs or
alcohol through any means are especially at risk, and those from mothers
on anti-epileptic medications are at very high risk and need special
attention.
Such complications reduce the effectiveness of vitamin K, and in these
cases, a higher level of available K could prevent the tragic
intracranial bleeding. This rare bleeding disorder has been found to be
highly preventable by a large-dose injection of vitamin K at birth.
The downside of this practice however is a possibly 80 percent increased
risk of developing childhood leukemia. While a few studies have refuted
this suggestion, several tightly controlled studies have shown this
correlation to be most likely. The most current analysis of six
different studies suggests it is a 10 percent or 20 percent increased
risk. This is still a significant number of avoidable cancers.
Apparently the cell division that continues to be quite rapid after
birth continues to depend on precise amounts of vitamin K to proceed at
the proper rate. Introduction of levels that are 20,000 times the
newborn level, the amount usually injected, can have devastating
consequences.
The Newborn's Diet
Nursing raises the infant's vitamin K levels very gradually after birth
so that no disregulation occurs that would encourage leukemia
development. Additionally, the clotting system of the healthy newborn is
well planned, and healthy breastfed infants do not suffer bleeding
complications, even without any supplementation.7
While breastfed infants demonstrate lower blood levels of vitamin K than
the "recommended" amount, they show no signs of vitamin K deficiency
(leading one to wonder where the "recommended" level for infants came
from). But with vitamin K injections at birth, harmful consequences of
some rare disorders can be averted.
Infant formulas are supplemented with high levels of vitamin K,
generally sufficient to prevent intracranial bleeding in the case of a
liver disorder and in some other rare bleeding disorders. Although
formula feeding is seen to increase overall childhood cancer rates by 80
percent, this is likely not related to the added vitamin K.
The Numbers
Extracting data from available literature reveals that there are 1.5
extra cases of leukemia per 100,000 children due to vitamin K
injections, and 1.8 more permanent injuries or deaths per 100,000 due to
brain bleeding without injections. Adding the risk of infection or
damage from the injections, including a local skin disease called "scleroderma"
that is seen rarely with K injections,8 and even adding the possibility
of healthy survival from leukemia, the scales remain tipped toward
breastfed infants receiving a prophylactic vitamin K supplementation.
However, there are better options than the .5- or 1-milligram injections
typically given to newborns.
A Better Solution
The breastfed infant can be supplemented with several low oral doses of
liquid vitamin K9 (possibly 200 micrograms per week for five weeks,
totaling 1 milligram, even more gradual introduction may be better).
Alternatively, the nursing mother can take vitamin K supplements daily
or twice weekly for 10 weeks. (Supplementation of the pregnant mother
does not alter fetal levels but supplementation of the nursing mother
does increase breastmilk and infant levels.)
Either of these provides a much safer rate of vitamin K supplementation.
Maternal supplementation of 2.5 mg per day, recommended by one author,
provides a higher level of vitamin K through breastmilk than does
formula, and may be much more than necessary.
Formula provides 10 times the U.S. recommended daily allowance, and this
RDA is about two times the level in unsupplemented human milk. One
milligram per day for 10 weeks for mother provides a cumulative extra 1
milligram to her infant over the important period and seems reasonable.
Neither mother nor infant require supplementation if the infant is
injected at birth.
The Bottom Line
There is no overwhelming reason to discontinue this routine prophylactic
injection for breastfed infants. Providing information about
alternatives to allow informed parents to refuse would be reasonable.
These parents may then decide to provide some gradual supplementation,
or, for an entirely healthy term infant, they may simply provide
diligent watchfulness for any signs of jaundice (yellowing of eyes or
skin) or easy bleeding.
There appears to be no harm in supplementing this vitamin in a gradual
manner however. Currently, injections are provided to infants intended
for formula feeding as well, although there appears to be no need as
formula provides good gradual supplementation. Discontinuing routine
injections for this group alone could reduce cases of leukemia.
One more curious look at childhood leukemia is the finding that when any
nation lowers its rate of infant deaths, their rate of childhood
leukemia increases. Vitamin K injections may be responsible for some
part of this number, but other factors are surely involved, about which
we can only speculate.

Study Finds Flaws in Prostate Cancer Test

BOSTON (AP) - A new study found that the standard screening test for
prostate cancer misses 15 percent of older men's tumors, including some
aggressive ones, when interpreted under existing guidelines.
The findings have sparked lively debate among doctors on how to
interpret the PSA test and how vigorously to treat screened men who show
no symptoms.
"We desperately need a new marker to tell us who needs to be treated
aggressively," said Dr. H. Ballentine Carter, a prostate cancer
researcher at Johns Hopkins University in Baltimore.
Some experts, like Dr. Leonard Gomella at Jefferson Medical College in
Philadelphia, think the threshold for what constitutes normal on the PSA
test could be lowered, at least in some cases. "It's a very powerful
test, but it's not perfect," he said.
But others, like Carter, say that could lead to more unnecessary
operations in the many men whose tumors are so slow-growing that
something else will kill them before the cancer ever does.
Sixteen percent of American men can expect to be diagnosed with prostate
cancer at some point in their lives. Yet most such tumors grow slowly,
with the death risk at just 3 percent. Existing screening methods cannot
always establish whether cancer is present and dangerous, so some cases
are missed and others are overtreated with surgery or radiation.
The study, conducted with the help of funding and personnel from the
National Cancer Institute, appears in Thursday's New England Journal of
Medicine.
It focuses on the standard screening test for prostate cancer: the
prostate-specific antigen test, or PSA count. The blood test has been
used on millions of men since the late 1980s to screen those with no
symptoms. Many start screening at age 50.
The test measures bloodstream levels of a protein manufactured by the
prostate, a male sex gland. Cancer expands the gland, pumping out more
of the protein and raising the PSA count. A count of 4 or below
(calculated in nanograms per milliliter) has been widely considered to
be normal.
However, the researchers found that 15 percent of 2,950 men ages 62 to
91 - all with normal PSA counts and rectal exams - had prostate cancer
anyway. And 2 percent of the overall group had tumors that looked
aggressive under a microscope.
"This study adds to information that perhaps the PSA threshold may be
dropped to 2.5 or so," said Gomella, the Philadelphia urologist. "The
number 4 may not be the, quote, normal that we look at anymore."
Lead study author Dr. Ian Thompson of the University of Texas at San
Antonio said the findings justify stronger measures for some men who
have low PSAs but other risk factors, such as prostate cancer in the
family.
However, other patients may decide more often to watch and wait, since
the findings - viewed in another light - add to the evidence that
harmless prostate cancer is quite common, Thompson suggested.
"It will allow two men to look at these data and come to different
conclusions," he said.
The stakes are high in doing too little. Cancer of the prostate kills
more men than any other kind except lung cancer. About 230,900 cases
will be diagnosed this year among American men, and about 29,900 will
die from it during the same period, according to the American Cancer
Society.
There is also evidence that prostate cancer screening is helpful. In the
late 1990s, 3 percent more cases were found and 4 percent fewer deaths
occurred each year, according to the most recent government data.
However, there is also potential harm in doing too much. An elevated PSA
count often stems from slow-growing and ultimately harmless tumors,
non-cancerous prostate enlargement, or infection. Prostate cancer
surgery leaves many men incontinent or impotent. And biopsies can lead
to bleeding or infection in unusual cases.
Also, it has not been conclusively proved that screening explains the
falling death rate. Maybe better, not earlier, treatments are saving the
lives.
Some doctors fear the study findings will lead to even more unnecessary
operations in older patients who are more likely to die of other causes
before the cancer kills them.
"I have a sneaking suspicion that what's going to happen is that people
are going to start ... being more aggressive with low PSAs," said Dr.
Siu-Long Yao, a prostate cancer specialist at the Cancer Institute of
New Jersey. "I don't think it's warranted at the present time, but I
think people will act on it."
Researchers are studying more subtle ways to interpret PSA counts and
experimenting with other protein and genetic tests to help pinpoint the
dangerous tumors.

are packed with fiber and folate, which are touted to be great at
reducing the risk of heart disease. Black beans, as all dried beans, are
also good sources of starches, fiber, B vitamins, iron, zinc,
phosphorus, complex carbohydrates and calcium. About half of the
calcium is lost during cooking. High percentages of the other nutrients
remain however, even after cooking.
All legumes are high in protein, and black beans are no exception. Dried
beans are important sources of protein in vegetarian diets, and in areas
where animal protein is scarce or expensive. However, this protein is
incomplete (does not contain all 9 amino acids), so grains (which
provide the missing amino acids) must also be a significant part of the
strictly vegetarian diet. In the areas where common beans originated
(Central America and southern Mexico) corn supplied the missing amino
acids, and squash was an additional source of vitamins.
Before cooking, be sure to pick through them, picking out any small
pebbles, split and withered beans and any other foreign matter. (Beans
from the Rockies and Pacific coast tend to have more adobe (bits of
clay) and stones). It is also helpful to cover the beans with cold
water, let sit for 5 minutes and remove anything that floats. Repeat to
be sure all dirt and foreign matter is removed. Drain and ready to cook
Here is one great way of eating black beans. Bean, Barley & Avocado Summer Salad
This can be refrigerated and stored for several days!
1 cup carrot juice
1/2 teasp. Thyme
1/2 teasp. Sea salt
1/8 teasp. Ground Cayenne pepper
1.2 cup quick-cooking barley
3 Tablespoons Lime or Lemon Juice
1 Tablespoon Virgin Olive Oil
1 can (9 oz) black beans, rinsed and drained
1 cup diced tomatoes
1/2 cup diced avocado
Combine carrot juice, thyme, sea salt and cayenne in a saucepan. Bring
to boil over medium heat; add barley and reduce to a simmer. Cover with
lid and simmer until barley is cooked, approx. 15 minutes.
While beans are simmering whisk lime/lemon juice and oil in a large
bowl. Add barley when cooked and any liquid remaining in the barley.
Toss to coat all.
Add beans, tomatoes and avocado, toss to mix well. Serve on a lettuce
leaf.

potassium-sparing diuretics:
These classes of meds, both used to lower blood pressure, may
raise cholesterol and/or
blood sugar. They're sold under many brand names.
2) Anabolic steroids and transdermal testosterone patches.
Anabolic steroids, used in treating specific types of anemia,
some breast cancers, osteoporosis, endometriosis, and hereditary
angioedema (swelling of the body). Anabolic steroids are
sometimes illegally and unsafely used by athletes to build
muscle, is also able to raise cholesterol levels. So can
transdermal testosterone patches, prescribed to
replace natural testosterone in males who have conditions that
cause a decrease in or absence of testosterone, such as delayed
onset of puberty.
3) Any medication that creates a change in the liver functions
and that is almost all of them!
So that cholesterol reading could be way off if you are taking
prescriptions or most OTC drugs.
Sometime in the next twenty years they will release, now
existing, reports that will say
cholesterol elevation was the wrong idea behind heart disease.
Of course that will most probably come when the drug company's
have found another medication to make them as much money as the
statin drugs do now.

Bacteria Found Living in Toxic Residue

YAKIMA, Wash. (AP) - Scientists have discovered bacteria living in the
toxic sediment beneath underground tanks that have leaked radioactive
waste at the Hanford nuclear reservation, home to some of the most
highly contaminated soil in the world.
The discovery eventually could help researchers better understand how
microorganisms can survive severe contaminants and how to use the
bacteria to help clean up toxic environments.
The results of a study of the bacteria were to be presented Wednesday at
the American Society of Microbiology's annual meeting in New Orleans.
"It's exciting," said Fred Brockman, staff scientist and group leader
for the project at the Pacific Northwest National Laboratory, an Energy
Department research center near the Hanford site in south-central
Washington. "One of the most important things to realize is this is a
type of environment that hasn't been studied with regard to bacteria.
"I believe it's the most radioactive soil ever studied with regards to
bacteria in the world," Brockman said.
The team of scientists has identified more than 100 different species of
bacteria in a mixture of sand, silt and clay extracted from beneath
several underground Hanford tanks, which were built in 1953 to hold
highly radioactive waste produced from the recovery of plutonium from
irradiated nuclear reactor fuel rods.
About 53 million gallons of radioactive liquid, sludge and other
material are stored in the 177 underground tanks. Some of the tanks,
which are less than 10 miles from the Columbia River, have leaked into
the groundwater, exposing the sediment for decades to very high
radioactivity.
Scientists cultured bacteria from the sediment and analyzed their DNA to
try to identify them.
"The proteins we found were quite dissimilar to what's already in the
international databases," Brockman said. "It's telling us that these
organisms just haven't been studied much before."
The findings also could be important to further study into using
organisms to clean contaminated sites.
"The key is that some of them have survived, and if we study them
further, we may be able to understand their particular properties or the
capabilities of the organism," Brockman said.
Hanford was an important site for the top-secret Manhattan Project to
build the atomic bomb during World War II. For 40 years, it processed
plutonium for the nation's nuclear weapons arsenal. Today, work there
centers on a $50 billion to $60 billion cleanup, to be finished by 2035.
The bacteria research was conducted as part of the Hanford Science and
Technology Program, aimed at understanding basic science critical for
cleanup applications.
