Tuberculosis Linked to Arthritis Drugs
Doctors Should Screen for TB Before Prescribing Drugs That Suppress
Immune System
Several cases of tuberculosis (TB) have been linked to drugs commonly
prescribed for arthritis, the CDC reports.
Doctors who prescribe potent immune-suppressing drugs for their
rheumatoid arthritis patients should screen them for TB exposure and
infection before prescribing these drugs, the CDC says. Tuberculosis is
common in certain settings like jails, prisons, homeless shelters,
chronic-care facilities, and in some countries.
The arthritis drugs are all in a class called tumor necrosis
factor-alpha (TNF-a) antagonist drugs and include Remicade, Enbrel, and
Humira. These drugs work by blocking TNF-a, a chemical that triggers
inflammation. They are approved for treating rheumatoid arthritis and
other autoimmune diseases where inflammation is a key component of the
illness.
This same inflammatory chemical -- TNF-a -- is also associated with
certain infectious diseases like tuberculosis. Blocking this chemical
can allow a latent TB infection to emerge, says the CDC report.
Tuberculosis is caused by bacteria that mainly attack the lungs. Often
the body can fight a TB infection, but the bacteria remain dormant in
the body.
TNF-a blocker medications suppress the immune system and can allow the
bacteria to re-emerge and cause tuberculosis. This is called a latent
infection.
New Cases Pop Up
In 2002, three cases of tuberculosis occurred in people taking Remicade.
Nine more cases have been reported in California. According to the CDC's
report, most of the reported cases are latent infections.
Details on the infected people:
The average age is 55, and 67% are female.
Fever, cough, weight loss, and enlarged lymph nodes are initial
symptoms. Eleven developed tuberculosis after receiving Remicade.
Eleven of the patients had been in countries where tuberculosis is
common or had contact with someone infected with tuberculosis. Eight
were taking other medications that suppressed the immune system.
Some case studies:
A U.S.-born, 55-year-old man with rheumatoid arthritis was diagnosed
with tuberculosis within 17 months of starting Remicade. Although his
arthritis had improved, he developed fever, lost weight, and developed
an enlarged lymph node. He then had a chest X-ray, which showed evidence
of TB. He was treated with medications but has since died of lung cancer
(he was a longtime smoker).
The second case involved a 64-year-old woman with rheumatoid arthritis;
she was diagnosed with tuberculosis just 10 months after starting
Remicade. Her symptoms were fever and weight loss. After 12 months of
medication, her medical condition has improved.
A third case involved a U.S.-born, 54-year-old woman with Crohn's
disease. She was exposed to tuberculosis in 1996 and had a positive skin
test for TB. She developed cough, fever, and stomach pains after
Remicade infusions in 2001. Her chest X-ray revealed tuberculosis. She
tried taking a standard, four-drug TB therapy but developed stomach
problems from it. Other tuberculosis drug treatments, however, worked.

rivers are already overstressed, yet there is a lot of complacency about
the future," the IWMI report says.
Anders Berntell, the director of the International Water Institute,
based in Stockholm, said: "The world's future water supply is a problem
that's ... greater than we've begun to realize.
"We've got to reduce the amount of water we devote to growing food. The
world is simply running out of water."

MMR VACCINE PROBLEMS IN BRAZIL

Lawrence Altman of the New York Times reports that Brazilian health
officials have stopped using Chiron's vaccine against measles, mumps and
rubella after "an unexpectedly high number of children who received it
experienced serious allergic reactions in an immunization program last
week. The reactions included rashes and anaphylactic shock, a
potentially fatal allergic condition." At least 125 children experienced
the reactions.
The experience in Brazil offers lessons here in the U.S. While
scientific studies have not established a link between MMR and autism,
other side effects from MMR are well documented. Even pro-vaccination
organizations acknowledge this. Meanwhile, measles, mumps and rubella
are extremely rare in the U.S. These diseases usually do not lead to
horrible adverse health outcomes.

Company Is Investigating Possible Vaccine Problems in Brazil

A day after the Chiron Corporation said it was delaying release of its
influenza vaccine in this country because some lots were contaminated,
the company confirmed that it was investigating possible problems with
use of a different vaccine in Brazil.
Brazilian health officials stopped the use of Chiron's triple vaccine
against measles, mumps and rubella, often referred to as MMR, after an
unexpectedly high number of children who received it experienced serious
allergic reactions in an immunization program last week. The reactions
included rashes and anaphylactic shock, a potentially fatal allergic
condition. There were no deaths reported.
Chiron and Brazilian health officials are investigating the cases of at
least 125 children who experienced the reactions.
The vaccine problems raise concern because Chiron, the world's
fifth-largest vaccine manufacturer, is under contract with the United
States government to produce pilot supplies of human vaccines against
two strains of avian influenza, which has spread widely in Asia. The
pilot vaccines are needed because health officials around the world have
expressed fears that in a worst-case scenario, the avian strains could
mutate to cause a human pandemic.

colonists from Florida to Canada. Today cukes grow in a wide variety of
shapes and sizes, from the 1"-long ones sold as gherkins to mammoth
greenhouse varieties that can reach 20" or longer. Can be found in
grocery stores year round! Healthy and nutritionally sound.
The high water content in cukes make them a great diuretic for those of
you looking to lose weight and/or lower blood pressure. They also are
full of fiber making it perfect to keep your intestinal system moving
right along another great weight loss item as well as lowering blood
pressure.
Cukes are said to be great in helping in the treatment of arthritis
since it helps eliminate uric acid. Its liquid is also said to be a
rejuvenator and makes us feel and look youthful. Now who can't handle
that? If you want to try rejuvenating your skin either puree or slice a
cucumber and apply it on your face, let it sit for a half hour or so
then gently rinse off with warm mineral rich water (mineral rinse; 1
teaspoon liquid minerals to a couple quarts of warm filtered/purified
water). Cukes are touted to have the power to relax and alleviate
sunburn pain as well.

slices in a sandwich or you can even juice it along with other
vegetables. Some people like them steamed like squash but they lose most

raw cucumbers, why, I don't know? Great fast food! My husband and
grandchildren have been caught eating them straight off the vine in the
garden!
This week will find me canning pickling cucumbers to eat year round.

Singapore Aims to Lure 'Medical Tourists'

SINGAPORE (AP) - A private hospital in Singapore slashed prices
for a slew of common operations Friday as part of city-state's
drive to lure tourists and expatriates seeking inexpensive
health care.
So-called medical tourism has boomed in recent years, especially
in neighboring Malaysia and Thailand, with foreign patients
traveling both for those nations' famed beaches and routine
medical procedures that are often cheaper than in their home
countries.
Singapore is seeking to step up its challenge for the
increasingly lucrative market, and the government has set itself
the target of attracting up to 1 million overseas patients by
2012.
The private 380-bed Raffles Hospital is hoping to treat up to
300,000 foreign patients a year by then, said Dr. Loo Choon
Yong, executive chairman of the Raffles Medical Group, one of
Singapore's top health care providers.
Loo said Raffles was offering "prices that are competitive and
comparable to the Thai hospitals" at a news conference to launch
the hospital's new campaign.
He also said Singapore's reputation for being squeaky clean was
well-deserved.
"We can you guarantee you, when you take blood from our blood
bank in Singapore it is safe, 100 percent safe," Loo added. "We
can also assure you in Singapore, the level of training and
professionalism is among the best in the world."
The hospital is now discounting 10 standard operations and
procedures ranging from having a baby and treating a cataract to
undergoing a heart bypass.
For example, Raffles is now offering an angiogram - an X-ray
picture of blood vessels - for $1,052 before service tax of 5
percent.
Loo didn't say how much it had cost before, but did say the same
procedure would cost the equivalent of about $1,000 in Thailand.
The downtown hospital offers comprehensive packages for those
going under the knife - and for their family members who travel
with them.
Dr. Prem Kumar Nair, general manager of the group, said staff
could arrange airport transfers, book relatives into nearby
hotels, and even help to arrange a local tour.
"That's why they call it's medical tourism: we do everything,"
Nair said.
Wealthy Singapore has generally higher costs than neighboring
states, and its economy is largely geared toward providing
services for domestic businesses, and those from around the
region.
"Neighboring countries also aspire to have medical tourism and
all that. That's fine: this is a free world," said Loo. "We have
the safety factor that must surely be very, very important."
LENA'S COMMENT: Vacation Medicine anyone? It was only a
matter of time before Asian countries got caught up in our
insane medical world, and I predicted it about about fifteen
years ago when I watched Asian doctors come for American Health
Seminars in Southern California... It looks like Westernized
Medicine did take hold in the Asian countries, and you can watch
for longevity to decrease in the future as this generation takes
stronger hold of Westernized medicine and abandon's their long

Farmers' Almanac Predicts a Wild Winter

LEWISTON, Maine (AP) - Gas up the snowblower but don't put away your
umbrella: The Farmers' Almanac is predicting a wild winter with heavy
precipitation and dramatic temperature swings in the Northeast.
The northern Plains and Great Lakes will be snowy, the almanac says,
while it will be milder in the southern half of the country. The
Northeast will have unusually wet weather - either as rain or snow,
according to the almanac.
"The big thing is it's going to be a winter of extremes," said managing
editor Sandi Duncan, whose almanac hits newsstands Tuesday.
The weather formula used by the 188-year-old Farmers' Almanac is a
closely guarded secret. Prepared two years in advance, the forecasts are
based on sunspots, the position of the planets and tidal action of the
moon.
The National Weather Service questions the accuracy of forecasts made so
far in advance, but the almanac says it is right 80 percent of the time.
Some people use it for planning weddings and other outdoor events.
The almanac's forecast last winter was largely on the mark. It predicted
a cold winter and Mother Nature delivered a January deep freeze in the
Northeast.
The almanac also forecast a wet and cool summer. And that seems to ring
true in many places, including Maine.
This coming winter will get off to a cold start in the Pacific Northwest
and northern Plains, the almanac says. Milder weather is in store for
southern half of the nation, with near normal or balmy temperatures from
California to the Deep South, according to the almanac.
Later in 2005, the almanac predicts a wet spring for most of the country
and an active pattern for the Midwest's "Tornado Alley" in April and
June.
The Farmers' Almanac - not to be confused with the Old Farmer's Almanac
in New Hampshire - also offers up recipes, brainteasers, jokes,
gardening tips and trivia.
This year's edition also continues a focus by the almanac editors on
finding happiness through simplicity.
An article on "Putting the Joy Back into Christmas" noted that the
holiday buying binge leads to personal debt and an additional 5 million
to 7 million tons of extra waste in landfills.
"We're not saying don't give. Let's give things that are more
significant to the recipient," said editor Peter Geiger.
The editors expect to distribute 4.5 million copies of the three
versions of the almanac: the 200-page retail version, a shorter
promotional version and a Canadian version.
Last year, the editors launched a syndicated column that's in 100
newspapers, and the almanac's Web site remains popular.
Now the editors are exploring a new medium.
"Our goal is to have a family-oriented television show," Duncan said.
