ABSURDITY
The next time you read a news story about how cancer
is decreasingly a problem in the US, think again. California officials
have revealed that the US Veterans Administration (VA), which treats
many American veterans, has been deliberately withholding cancer
incidence data from state cancer registries across the country. This has
resulted in up to 70,000 newly diagnosed cancer cases per year - about 5
percent of the national total - going unrecorded in the past few years.
An internal report from the California cancer surveillance agency
reveals that the VA stopped reporting cancer cases to state registries
in late 2004. As a consequence of this policy, "statewide and
national data will be incomplete and inaccurate," says Kurt Snipes
of the Cancer Surveillance Branch of the California Department of Health
Services in Sacramento.
VA officials admit that they are withholding cancer incidence data, but
argue that they are doing so to protect veterans' privacy rights. Yet,
oddly, they continue to report non-cancer diagnoses, including HIV/AIDS.
The VA has also refused to allow state health officials to conduct
routine audits of cases at VA hospitals.
According to Reda Wilson of the Centers for Disease Control (CDC) in
Atlanta, VA hospitals in at least 13 states are no longer reporting
cancer cases and VA reporting has been "inconsistent" in an additional
14 states. Furthermore, Florida's cancer registry has never received any
VA cancer data at all. The VA itself admits that 29 veterans hospitals
withheld cancer data in 2006.
Dr. Wilson says that this deliberate underreporting has resulted in
somewhere between 40,000 and 70,000 cases being potentially missed
nationally each year. As a result, the official figures for the annual
US incidence of major killers such as prostate, lung and colorectal
cancer may all now be significantly underestimated.
The National Cancer Institute (NCI) and other national surveillance
organizations admit that nationwide cancer rate estimates next year will
be artificially low because of the VA's omission. According to
officials, the omission could introduce "uncorrectable bias" into future
epidemiological studies. "Research from the mid-2000s will forever
require an asterisk, or perhaps a sticker on the cover, to remind
researchers and the public that they are not correct." All efforts
by scientists at the NCI and elsewhere to prevent this policy from being
allowed to introduce such glaring bias have failed, and the US
Department of Health and Human Services (HHS) has refused to intervene.
"We've been working with the VA for more than 5 years, but it's just
gotten worse," said Holly Howe of the North American Association of
Central Cancer Registries.
The VA has replied with unbelievable defensiveness to the charges. "It
is beginning to sound like a witch hunt by the national [cancer
surveillance] standards setters to punish the VA for not subordinating
itself to them," said Raye-Ann Dorn, the VA's national coordinator
for cancer programs. "Their primary beef is that the VA said no to
their strong arm tactics and has the audacity to protect our patients'
private health information from inappropriate invasions of their
privacy," she said.
I have rarely heard such an implausible explanation from any government
official. Witch hunt? Strong arm tactics? We're talking about cancer
statistics here, numbers on which crucial public policy decisions and
recommendations necessarily depend.
So what's really going on? Is this just bureaucratic trench warfare or
is there are some hidden political agenda at work? No one knows. But let
us for the moment apply the Roman principle of "qui bono?" That is to
say, let us ask who might benefit from this otherwise inexplicably
stupid action? Low numbers generally are taken as a sign of progress in
the war on cancer. It now appears that for years someone has been
fiddling with the record books, quietly reducing the overall number of
cancer cases. Inevitably the apparent drop in cancer incidence lends
credibility to the frequently-uttered assurances of steady progress in
the war on cancer.
In mid-January, for instance, Pres. Bush went to the NCI, where he
publicly took credit for much smaller changes in the cancer statistics.
Taking advantage of a drop of just one-half of one percent in cancer
mortality between 2003 and 2004 he intoned: "Progress is being made."
Bush also characterized this tiny decline in mortality as "the
steepest drop ever recorded." One can only imagine what current
administration spokespersons will make of an apparent 5 percent decline
in incidence.
The VA itself is currently completing a study of cancer in Gulf War
veterans. In addition, media reports have begun to raise concerns about
possibly increased cancer rates in Iraq war veterans. But, ironically,
the VA is basing its study on - you guessed it - the very state cancer
registries from which it deliberately withheld data. Some state
officials have therefore refused to participate in the VA's Gulf War
study, because they don't believe in the validity of their own data.
The NCI and state cancer registries are now attempting to introduce some
statistical corrections into the data in order to accommodate these
missing veteran cancers and minimize the impact of their omission on
estimates of US cancer rates. What a situation, when one arm of the
government has to work out complicated formulas to correct for errors
and omissions committed by other agencies.

Deaths Blamed on Improper
Painkiller Use

WASHINGTON (AP) - The deaths of two patients prescribed a powerful painkiller as
a headache treatment were among four fatalities linked to the recently approved
drug, its manufacturer reported Thursday.
All four deaths apparently involved improper use of the drug, called Fentora,
manufacturer Cephalon Inc. (CEPH) said. The Food and Drug Administration was
monitoring the situation, a spokeswoman said.
The FDA approved the drug in September for use only by cancer patients already
taking morphine or other prescription narcotics for their pain. Fentora contains
fentanyl, which is similar to morphine, but far more potent.
Besides the two headache patients, the other deaths involved a suicide and a
patient administered the drug outside the recommended dosing.
"None of the reports were in cancer patients, which leads us to believe they
were inappropriate candidates for the product," Cephalon spokeswoman Candace
Steele said.
Cephalon reminded doctors and pharmacists, in letters dated Monday but released
Thursday by the FDA, of who should take the drug, and in what quantities and how
often. The company also warned not to substitute Fentora on a one-for-one basis
for another, older fentanyl drug called Actiq that it also makes, because the
newer drug is stronger.
"The FDA takes this very seriously, and is working with the company to assure
the safest possible use of this medication," agency spokeswoman Susan Cruzan
said.
Steele said the deaths were not linked to allegations subject to investigation
that Cephalon engaged in improper promotion of Fentora and other drugs for uses
other than those outlined in their FDA-approved labels. While it's not illegal
for doctors to prescribe drugs for so-called "off-label" uses, companies are
prohibited from marketing medicines for those purposes.
Last month, Frazer, Pa.-based Cephalon said it was cooperating with the
investigations by Congress, the U.S. attorney's office in Philadelphia and
Connecticut attorney general.
Doctors have written about 78,000 prescriptions for Fentora tablets since
October, Steele said.

Is Nicotine
Replacement Really Safe?
(Ivanhoe Newswire) -- We know nicotine is
what makes cigarettes so addictive, but it's all that other
stuff in cigarettes that makes them so deadly, right?
Researchers who studied the effects of lower and higher nicotine
cigarette smoke in mice report we might have to adjust those
assumptions. Their study reveals mice exposed to lower-nicotine
smoke developed smaller atherosclerotic lesions than mice
exposed to higher-nicotine smoke. The finding suggests nicotine
replacement products like the gum and patch might not be as safe
as previously reported.
"Right now, the general consensus is that the problem with
cigarettes is tar and that nicotine is safe. That's why you can
buy nicotine gum or patches to help you stop smoking," Dr.
Daniel F. Catanzaro, principal investigator of the study, was
quoted as saying. "Our study presents new evidence that nicotine
may not be safe at all, especially for your heart."
He and his colleagues compared atherosclerotic lesions in mice
exposed to smoke from a new type of low nicotine cigarette that
works by heating up inhaled air to activate its contents rather
than actually burning the tobacco. Another new type of cigarette
containing tobacco that's been genetically engineered to have a
lower level of nicotine was also used in the study. These
cigarettes were compared to traditional cigarettes with higher
levels of nicotine. All the cigarettes contained about the same
amount of tar.
While mice exposed to smoke with lower levels of nicotine
developed smaller lesions, the investigators stress the best
outcomes were seen for control mice, which weren't exposed to
cigarette smoke at all. Researchers report this finding
emphasizes what doctors have been telling the public for years:
don't smoke, and if you do, quit.
Study authors also note switching to a cigarette with lower
levels of nicotine might not do much good in real life, as
previous research has suggested people who switch from regular
cigarettes to lower tar and nicotine cigarettes usually just end
up smoking more and taking deeper drags to compensate.

Melt Opens Northwest Passage

PARIS (AP) - Arctic ice has shrunk to the lowest level on
record, new satellite images show, raising the possibility that
the Northwest Passage that eluded famous explorers will become
an open shipping lane.
The European Space Agency said nearly 200 satellite photos this
month taken together showed an ice-free passage along northern
Canada, Alaska and Greenland, and ice retreating to its lowest
level since such images were first taken in 1978.
The waters are exposing unexplored resources, and vessels could
trim thousands of miles from Europe to Asia by bypassing the
Panama Canal. The seasonal ebb and flow of ice levels has
already opened up a slim summer window for ships.
Leif Toudal Pedersen, of the Danish National Space Center, said
that Arctic ice has shrunk to some 1 million square miles. The
previous low was 1.5 million square miles, in 2005.
"The strong reduction in just one year certainly raises flags
that the ice (in summer) may disappear much sooner than
expected," Pedersen said in an ESA statement posted on its Web
site Friday.
Pedersen said the extreme retreat this year suggested the
passage could fully open sooner than expected - but ESA did not
say when that might be. Efforts to contact ESA officials in
Paris and Noordwik, the Netherlands, were unsuccessful Saturday.
A U.N. panel on climate change has predicted that polar regions
could be virtually free of ice by the summer of 2070 because of
rising temperatures and sea ice decline, ESA noted.
Russia, Norway, Denmark, Canada and the United States are among
countries in a race to secure rights to the Arctic that heated
up last month when Russia sent two small submarines to plant its
national flag under the North Pole. A U.S. study has suggested
as much as 25 percent of the world's undiscovered oil and gas
could be hidden in the area.
Environmentalists fear increased maritime traffic and efforts to
tap natural resources in the area could one day lead to oil
spills and harm regional wildlife.
Until now, the passage has been expected to remain closed even
during reduced ice cover by multiyear ice pack - sea ice that
remains through one or more summers, ESA said.
Researcher Claes Ragner of Norway's Fridtjof Nansen Institute,
which works on Arctic environmental and political issues, said
for now, the new opening has only symbolic meaning for the
future of sea transport.
"Routes between Scandinavia and Japan could be almost halved,
and a stable and reliable route would mean a lot to certain
regions," he said by phone. But even if the passage is opening
up and polar ice continues to melt, it will take years for such
routes to be regular, he said.
"It won't be ice-free all year around and it won't be a stable
route all year," Ragner said. "The greatest wish for sea
transportation is streamlined and stable routes."
"Shorter transport routes means less pollution if you can ship
products from A to B on the shortest route," he said, "but the
fact that the polar ice is melting away is not good for the
world in that we're losing the Arctic and the animal life
there."
The opening observed this week was not the most direct waterway,
ESA said. That would be through northern Canada along the coast
of Siberia, which remains partially blocked.
