Physicians Push FDA to Drop 'Recall' Use

WASHINGTON (AP) - The Food and Drug Administration is considering not using the word "recall" to warn patients and doctors about defective pacemakers and defibrillators at the request of a physicians' group struggling to deal with a loss of public confidence in the safety of implantable heart devices.

FDA officials say they are conducting focus groups to see whether the currently used "recall" term is causing undue alarm when used to refer to heart devices the agency has decided might be faulty.

"It's a terrible term," said Dwight Reynolds, president of the Heart Rhythm Society, an association of doctors who implant the devices. "The anxiety created among patients and physicians by this term is the No. 1 cause for replacement of devices." According to Reynolds, patients who learn they have a faulty device often assume they need to have it removed immediately, even though the surgical procedure to replace the device typically involves more risk than leaving it in.

The Heart Rhythm Society released guidelines today, asking the FDA to use "safety advisory" or "safety alert" when referring to device problems.

The FDA has previously said eliminating the "recall" term would require changing federal law, but physicians and manufacturers disagree. They met with lawmakers this week to see whether a non-legislative solution could be negotiated with regulators.

The request faces obstacles, however, since "the term 'recall' is used across many different kinds of products" at the FDA, says Bram Zuckerman, head of the agency's cardiovascular device division.

Industry leaders Medtronic Inc. (MDT), Boston Scientific Corp. (BSX) and St. Jude Medical Inc. (STJ) have already adopted many of the society's other recommendations, which appeared in draft form in April. The companies have said they will simplify how they communicate device performance, appoint external panels to review device problems and implement wireless systems that allow doctors to remotely monitor patients.

The FDA has issued safety alerts and recalls affecting more than 337,000 defibrillators since 1990. On average, less than 5 in 1,000 pacemakers and 21 in 1,000 defibrillators have had malfunctions resulting in replacement.

The number of defibrillators implanted in the U.S. has shrunk 8 percent in the last 12 months, according to Prudential Equity Group. The drop comes after a decade when defibrillator use expanded from 21,000 patients in 1995 to 250,000 last year. Industry analysts blame the slump on a series of highly publicized device safety problems that occurred last year.

The most controversial case involved Guidant Corp.'s Prizm defibrillators. Guidant, since acquired by Boston Scientific, fixed the problem in 2002 but did not inform patients and physicians until after the 2005 death of a patient with one of the devices.

Implanted in the upper chest, defibrillators monitor the heart for deadly irregular heartbeats and use electrical shots to jolt it back to normal. They differ from pacemakers, which use lower-voltage electrical pulses to correct dangerously slow heart rhythms.

Shares of Boston Scientific fell 4 cents to close at $14.77 on the New York Stock Exchange, close to the bottom of a 52-week range of $14.43 to $27.82.

Shares of Medtronic increased 38 cents to $46.35, finishing near the low end of the range for the past year of $42.37 to $59.87. St. Jude rose 33 cents to end at $35.50, also the low end of the $31.20-to-$54.75 range of the past year. Both companies trade on the NYSE.

LENA'S COMMENT: This looks rather suspicious? What are they thinking? When is it okay for stock investors to be more important than a person's life or peace of mind? Never, should be the answer but I guess it's not...

A baby's digestive system isn't fully developed, and some infants have a hard time absorbing sorbitol. If these infants are given a juice with a lot of sorbitol, their metabolic rate increases, as does their level of physical activity. That's not always a good idea. The juice with the most sorbitol is pear juice, so you might want to hold back on that for a while. White grape juice is probably the safest to start a baby on.

The Fruit That Played A Role In Knights Of Old!
 
As my husband handed me the healthy gold fruit, from our tree, I knew what the following food of the week column was going to be about?
 
These sweet juicy yellow, green and sometimes a musty brown fruit with a rounded shape that becomes narrower towards the stalk and has kind of a grainy texture that turns some people off. Pears offer a healthy snack as well as a healthy ingredient in many recipes.  Full of natural nutritious fiber!

 
Pear History is interesting in that pears date back to the early Asian history!
 
Pears are one of the world's oldest cultivated and beloved fruits. In 5,000 B.C., Feng Li, a Chinese diplomat became consumed by grafting peaches, almonds, persimmons, pears and apples as a commercial venture. In The Odyssey, the Greek poet laureate Homer lauds pears as a "gift of the gods." Pomona, goddess of fruit, was a cherished member of the Roman Pantheon and Roman farmers documented extensive pear growing and grafting techniques. Thanks to their versatility and long storage life, pears were a valuable and much-desired commodity among the trading routes of the ancient world. Evident in the works of Renaissance Masters, pears have long been an elegant still-life muse for artists. In the 17th century a great flourishing of modern pear variety cultivation began taking place in Europe. And in popular culture, the pear tree was immortalized alongside a partridge in the 18th-century Christmas carol, The Twelve Days of Christmas.
 
Early colonists brought the first pear trees to America's eastern settlements where they thrived until crop blights proved too severe to sustain widespread cultivation. Fortunately, the pear trees brought west to Oregon and Washington by pioneers in the 1800's thrived in the unique agricultural conditions found in the Pacific Northwest. Today's Northwest pear varieties are the same or similar to those first cultivated in France and Belgium where they were prized for their delicate flavor, buttery texture, and long storage life.
 
Pears are picked when they are almost ripe. The fruit will come off if you lift the fruit vertically, signally time to pick. Pears ripen best at room temperature. If you want too keep them a few days you have to keep them in a dark cool place. There's no need to peel a pear... their tender, edible skin is an additional and nutritional source of fiber. A medium sized pear provides 4 grams of fiber, or 16% of the recommended daily value. Always wash all fresh fruits and vegetables before serving.
 
More levulose - the sweetest of known natural sugars - is found in fresh pears than any other fruit, which doesn't really sound like it's good for dieting. Well, levulose is easily digested and plays a role in energizing the metabolism thereby helping to bring about weight loss. The pear is rich in the minerals; Calcium, Iron, Magnesium, Phosphorus, Potassium - 30% more than an apple - , Sodium, Zinc, Copper, Manganese and Selenium Not short changed in the vitamin department either containing; Vitamin C, B-6, B-12, A, E, Rhiamin,  Riboflavin, Niacin, Pantothenic acid, Folate and Folic acid.  
 
There are many varieties of pears to choose from and should hit just about any desire!
 
*Yellow Bartlett (Bart-let) U.S. Availability in August through January ripens to bright yellow, perfect for fresh eating. Very sweet and juicy. Excellent for canning or cooking. 
 
*Red Bartlett (Bart-let) Availability in August through January Bright red skin when fully ripe. Same flavor, texture and use as yellow Bartletts.
 
* Green Anjou (On-ju) Availability in October through June. Abundant juice and sweet flavor when ripe. They do not change color as they ripen.
 
* Red Anjou (On-ju) Availability in October through May. Much the same flavor and texture as green Anjous. Remains maroon red when ripe.
 
* Bosc (Bahsk) Availability in August through April. Highly aromatic flavorful pear. Dense flesh makes it ideal for baking and cooking. They are brown and often russeted. They do not change color as they ripen. 
 
* Comice (Co-meece) Availability in August through February. One of the sweetest, juiciest varieties, and often are very large. An elegant dessert pear that's excellent with cheese. Almost no color change when ripe. 
 
* Seckel (Seck׳l) Availability in August through February. Tiny pears with ultra-sweet flavor, maroon and olive green in color. Excellent choice for children's snacks, pickling, or as a garnish. No color change when ripened.
 
* Forelle (For-ell) Availability in September through February. A smaller variety. Turns bright yellow with crimson freckling when ripe. Sweet, very juicy.
 
Our favorite way of eating pears is fresh and raw but poached pears are excellent as well and here is a relish recipe that is very tasty and nourishing.
 
True Sugar-free Pear Relish
 
Great on toast, English muffins or as a relish for meats.
 
      8  quarts of Pears
      6  Onions
      3  Red bell peppers
      3  Green bell peppers
      5  cups apple cider Vinegar
      2  teaspoons Sea Salt
      1  teaspoon Pickling spices
      1  teaspoon Turmeric
      Stevia: 8 Tablespoons. whole Stevia leaf powder  or 1/4 cup white Stevia extract (powder)
 
Grind vegetables. Combine with remaining ingredients; bring to a boil and boil until thickened, about 30 minutes. Seal in pint jars. Makes about 8 pints.

Dozing off without a prescription - Americans are exploring alternatives to drugs, a new study finds -- and discovering they help.

Whether meditating before bed or sipping a kava kava nightcap, more than 1.6 million Americans use some form of alternative medicine when they have trouble sleeping.

In analyzing data from 31,000 Americans interviewed for the 2002 National Health Interview Survey, researchers found that nearly one-fifth of adults reported difficulty sleeping in the last 12 months, and of those, about 5% used complementary and alternative medicine to treat their sleeplessness. The majority of those who tried the therapies said they helped, with nearly half saying they helped "a great deal."

 Nearly 65% of people using alternative methods to help them sleep used "biological therapies," such as herbs or supplements, and 39% used "mind-body therapies," such as self-hypnosis, guided imagery or other relaxation techniques.

The findings also shed some light on why people turn to complementary and alternative medicine to treat their sleep problems. Forty percent of those who tried alternative therapies said they had not found conventional treatments helpful. Thirty-five percent said their doctor had suggested the approach. One-quarter thought conventional medicine was too expensive, and two-thirds thought it would be "interesting to try."

The report, published last week in the Archives of Internal Medicine, was part of a larger look at the nation's sleep habits. Researchers also found a strong connection between reports of insomnia or sleep troubles and other health conditions, such as obesity, hypertension, congestive heart failure and anxiety or depression.

"This is giving us a nice snapshot of the characteristics of people who have insomnia in general, as well as those who use complementary and alternative medicine for insomnia and sleep problems," said Richard Nahin, senior advisor for scientific coordination and research at the National Center for Complementary and Alternative Medicine.

Although the report didn't rank the popularity of specific herbal or behavioral remedies, doctors who recommend complementary and alternative medicine said some alternative therapies ? such as melatonin, kava kava and valerian ? can be effective in treating sleep problems and are typically safer than sleep drugs.

"People are aware that a lot of conventional therapies do have side effects, and there is a potential for a dependency to develop," said Dr. Mary Hardy, director of integrative medicine at the Ted Mann Family Resource Center at UCLA's Jonsson Comprehensive Cancer Center. "If they want to be able to take something on a regular basis, they look to natural therapies to help with that."

Dr. Jay Udani, who runs the Integrative Medicine Program at the Northridge Hospital Medical Center, said he would recommend anyone with sleep problems start with mind-body techniques such as self-hypnosis, meditation or guided imagery. If that were not sufficient, he might recommend mind-body techniques combined with an herbal remedy ? and melatonin would be his first choice. Even a low dose (1 to 5 milligrams) of melatonin can be effective, he says.

But doctors cautioned against mixing sleep-inducing herbs or supplements with sleep drugs. Consumers should talk with their physicians first, they said.

Dr. Soram Khalsa, an internist at Cedars-Sinai Medical Center who has integrated natural and conventional medicine for the last 30 years, said he often recommends tryptophan and theanine, two amino acids that can be bought over the counter. He said peri-menopausal women who have a slight hormonal imbalance ? they are low in progesterone relative to estrogen ? might benefit from natural progesterone before bed. He also recommends meditating at night.

"If you meditate for even 15 minutes with some deep breathing, that will help induce the alpha rhythm brain wave which will help you sleep," he said.

Hardy said her recommendations depend on the sleep disorder. People who have trouble falling asleep because of anxiety should consider relaxation therapies, such as meditation, or calming yoga poses before bed, she says. Aromatherapy, such as lavender or geranium oils, can also help, she added, as can a pre-bedtime snack of foods that promote the release of seratonin, such as crackers, bread, tuna or a little milk.

Hops can be helpful, but valerian is the single best herbal medicine for sleep, Hardy said, because it doesn't excessively sedate, and over time it helps restore normal "sleep architecture." She said the herb is best for those who fall into a deep sleep, but wake up not feeling rested.

The new data will help generate more interest in complementary and alternative medicine and sleep disorders, Nahin said, particularly in the realm of mind-body therapies

Local Governments Get Serious About the Environment

In the absence of serious national action, state and local governments and the private sector are taking the initiative in confronting the interlocked problems of global warming and energy conservation.

The most dramatic example is the legislation California Gov. Arnold Schwarzenegger signed last week mandating reductions in the state's emissions of the gases associated with global warming. But city governments and the private sector are also formulating some of the most innovative responses to a problem the federal government still refuses to confront head-on.
