Don't be surprised if you see an Inspector Clouseau type lurking around the supplement aisles in your local health food store. He might be an FDA agent.

A few days ago, FDA officials announced some new initiatives designed to "refine the direction" of the agency in regulating dietary supplements. One of these initiatives is to send inspectors into retail stores, looking for supplement labels that bear unsubstantiated claims. (Cue the Pink Panther theme music.)

A Reuters Health report on the new initiatives states that supplements currently aren't required to pass "the strict safety and effectiveness testing required for pharmaceuticals before they can be sold."

Ah yes, I guess that would be a reference to the type of "strict" FDA safety testing for pharmaceuticals that allowed Vioxx to stay on the market for years after the drug was clearly proven to increase heart attack risk.

Don't bother trying to make sense of it. It's an FDA thing.

Birth-Control Shot Gets Black Box Warning

WASHINGTON (AP) - Depo-Provera, an injectable contraceptive, will come with a special warning that links prolonged use of the drug with bone density loss, the government said Wednesday.

The Food and Drug Administration's most strident warning will inform women that the drug can cause significant loss of bone density. Losses are greater when the drug is used longer. In addition, bone density loss may not reverse completely when women stop using Depo-Provera.

A spokesman for the drug maker said bone density loss has been "at least partially reversible" in girls and adult women who have used Depo-Provera.

Nearly 400 girls age 12 to 18 who were taking the drug were enrolled in a study in 1997 to try to restore lost bone mineral density. The girls will be monitored through late 2006, said Paul Fitzhenry, a Pfizer Inc. spokesman.

In letters to doctors who are likely to prescribe the contraceptive, Pfizer will explain the effect of long-term treatment on bone mineral density.

The National Osteoporosis Foundation already includes Depo-Provera among medications that lower bone mass, placing people at risk of developing osteoporosis.

The warning on the label will be surrounded by a black box to call attention to the caution.

The FDA's decision to add such a warning followed analyses by the agency and Pfizer of data about the drug's long-term effect on bone density among teens and adult women. The women's study ran from 1994 to earlier this year and enrolled 540 women aged 25 to 38, Fitzhenry said.

Also this week, the FDA said it would bolster existing black box warnings for RU-486 after a third woman died after using the abortion pill.

Depo-Provera stops ovaries from releasing eggs. Because the body no longer readies itself for pregnancy, it either builds a thin uterine lining or no lining at all. A single shot remains potent for three months.

Women who want long-term birth control should use Depo-Provera only if other contraceptives prove inadequate, the FDA said.

Pfizer and the FDA stood by the safety of the drug, approved in 1992.

The Bashful Bulb

Beets or beetroot! So bashful they keep their heads in the ground.
 
Some refer to this purplish wonder as beets others as beetroot. Either way is correct. Some people only eat the root bulb but the greens are very nutritional and tasty and one of my favorites.
 
Once you've tasted fresh beets in the peak of their season from June through October, you'll delight in their sweetness and versatility. I should mention that they have the highest sugar content of all the vegetable kingdom. There are factories that process the sugar from the beets, called Sugar Beet Factories!
 
Beets and beet greens contain the health building minerals calcium, Iron, magnesium, Phosphorus, Potassium, Sodium, Zinc, Copper, Manganese, Selenium along with the vitamins  C, B-6, B-12, A, E, Thiamin, Riboflavin, Niacin, Pantothenic acid, Folate, and Retinol.
 
Experts believe that the red pigmentation of beetroot contains certain anti-cancer agents. Beetroot has for many years been used as a treatment for cancer in Europe. Specific anti-carcinogens are bound to the redish/purple coloring, which supposedly helps fight against cancer and beetroot also increases the uptake of oxygen by as much as 400 percent. Additional studies are taking place to add support to these claims.
 
The green leafy part of the beetroot contains beta-carotene and other carotenoids,(Carotenoid refers to plant pigments - of which there is a family of about 600 different types. These all function as antioxidants. The yellow, orange, and many of the red pigments in fruits, vegetables, and plant materials are usually carotenoids.) The roots and greens are especially good for women during their menstrual period and those planning pregnancy.
 
Preparing raw beets requires a mindset; put on an apron, to catch the splatter and roll up your sleeves. Next, tell yourself that for that entire day you will be the proud owner of a pair of red hands that look like you just killed someone unless you wear gloves. Then, get to work on the beets.
 
For salads, cut off the root and stem ends, peel and coarsely grate the beets. Place them on the top of an individual salad as a garnish or serve them in a separate bowl tossed with seasoned rice vinegar to be passed at the table. If you toss the beets into the salad, the entire salad will blush. Sometimes this may be a desired effect when you want to give your veggies a rosy glow.
 
Beets are famous for blushing or, more commonly called bleeding. To reduce bleeding and preserve more of the flavor as well as the nutrients, cut beet tops off, leaving at least 1" of stems intact. Usually when you buy fresh beetroot it will still have the leaves and stalks attached. To cook the beetroot simply cut off the stalks making sure to leave some of the stalk in tact as it helps to hold in the nutrients.
 
Beetroot can be steamed or cooked in boiling water. Cooking time can be from 20 to 50 minutes depending on the size of the beetroot and your desire for soft or crunchy. Test the beetroot with a skewer: when it's soft to your desire, remove it from the heat and cool it under running water - this will make the skin easier to remove for serving.
 
Try raw:
Grated finely can add to other vegetables.
Mix grated beetroot with raspberry vinegar or seasoned rice vinegar.
Mix grated beetroot, grated orange rind and orange juice for a tasty salad.
Plain grated beetroot is great on hamburgers or veggie burgers.
 
You can serve cooked beetroot as a hot vegetable accompaniment to a meal; or allow it to cool and slice it to put on a homemade burger. Cut into cubes and stir-fry it with some steamed cubed potatoes and pumpkin. Add a little garlic and some diced onions ? this makes a delicious vegetable dish to serve with the rest of your meal.
Roasted: To roast the beets, its best to peel them and cut off the root and the stem ends. Slice them, toss or spray a tablespoon or two of Olive oil, and spread them out on a roasting pan. Roast at 400 for about 25 - 30 minutes, turning mid way through. To add a little pizzazz, when tossing them in the oil prior to roasting, sprinkle with salt and some dried thyme, dill, marjoram, or oregano ad onion chunks for more flavor.
 
Prepare the greens as close to the purchase time as possible to retain all those health benefits. Wash them thoroughly, and coarsely chop. Add enough water to cover the bottom of the saucepan by 1/2". Squeeze in the juice of one half lemon and add a little salt, onion powder, lemon pepper and a hint of cayenne. PACK greens into a smaller saucepan than you might think you need--one bunch of beet greens cooks down to practically nothing!  Cook uncovered over high heat, which allows some of the oxalic acid in the greens to escape, a desirable process, since oxalic acid inhibits the absorption of calcium contained in the greens.
 
Today's recipe is from a former patient whose mother won a Betty Crocker baking contest with it years ago and a lot of money, but she gave the recipe to me so don't worry about it being a copy write problem! This is oh so good and even my kids and grandkids eat it without knowing what's in it? Color is nice as well as delightful flavor?
 
Beet Cake

Oil and flour 2 round cake pans
Heat oven to 350?
 
Ingredients:
1 ½ cups olive oil
2 cups sugar
3 eggs
2 cups flour
2 teaspoons baking soda
3 teaspoons ground cinnamon
1 teaspoon salt
1 cup crushed pineapple (drained)
1 cup Flaked coconut
1 cup chopped nuts (pecan or walnuts)
1 cup cottage cheese
1 cup shoestring beats with juice
 
Begin by;
Beating with mixer olive oil, sugar, eggs until thick and lemon yellow;
 
Add to it flour, baking soda, ground cinnamon, salt mix for 2 to 3 minutes.
 
Add and Mix by hand the following
Crushed pineapple (drained), coconut, chopped nuts, cottage cheese
 
Mix well then add beats with juice and stir until mixed thoroughly and evenly colored.
 
Pour even amounts into 2 cake pans.
Put into oven and 350?, bake 1 hour or until cake springs back when touched lightly.
 
This is best frosted with the old fashioned Philadelphia Frosting;
 
Cream together the following ingredients for philly frosting'
1- box powdered sugar
1- stick (4 oz.) butter
1- tsp pure vanilla flavor
 
When creamed put one-fourth of the icing on the first cake layer and spread. Top that with second cake layer and frost whole cake or just the top, either way works well? refrigerate until ready to eat
.

FDA Called 'Defenseless' Against Bad Drugs

WASHINGTON (AP) - The American public is "virtually defenseless" if another medication such as Vioxx proves to be unsafe after it is approved for sale, a government drug safety reviewer told a congressional committee Thursday.

"I would argue that the FDA as currently configured is incapable of protecting America against another Vioxx," said David Graham, who warned that the arthritis drug had been linked to an increased risk of heart attack and stroke.

He told the Senate Finance Committee that there were at least five other drugs on the market today that should be looked at seriously to see whether they should remain there. He cited the acne drug Accutane, the weight loss drug Meridia, the anti-cholesterol drug Crestor, the pain reliever Bextra, and the asthma drug Serevent.

Vioxx's maker, Merck & Co. (MRK) pulled the drug from the market on Sept. 30 after a study indicated the popular painkiller doubled the risk of heart attacks and stroke when taken for longer than 18 months.

Raymond V. Gilmartin, the company president, said in prepared testimony that Merck acted within four days of learning about the risk.

"Given the availability of alternative therapies and the questions raised by the data withdrawing Vioxx was consistent with an ethic that has driven Merck actions and decisions for more than 100 years," he said.

Gilmartin also said the company was surprised by the cardiovascular risk because it differed from past clinical trials. "My wife was a user of Vioxx until the day we withdrew it from the marketplace," he said.

The Food and Drug Administration has defended its actions regarding Vioxx. In a statement issued late Wednesday, the agency cited its "well-documented and long-standing commitment to openness and transparency in its review of marketed drugs."

However, Sen. Charles Grassley, R-Iowa, who chaired the hearing, suggested that an independent board of drug safety might be needed to ensure the safety of medications after they're approved for the market.

"Consumers should not have to second-guess the safety of what's in their medicine cabinet," he said.

Graham told the committee that research indicated that Vioxx caused up to 160,000 heart attacks and strokes.

"If we were talking about Florida or Pennsylvania, 1 percent of the entire state population would have been affected," he said. "I'm sorry to say Sen. Grassley, but 67 percent of the citizens of Des Moines would be affected and, what's worse - the entire population of every other city in the state of Iowa."

Graham said his research helped to coax the FDA to withdraw a number of drugs including Fen-phen, a weight loss drug, Lotronex, Baycol and Rezulin. "During my career I have recommended the market withdrawal of 12 drugs," he said. "Only two of these remain on the market today."

At the same time, though, he questioned the agency's commitment to removing unsafe drugs from the market, since it would call into question their earlier approval.

Sen. Jeff Bingaman, D-New Mexico, said the problem was within the FDA's own culture.

"The culture within the FDA, being one where the pharmaceutical industry, which the FDA is supposed to regulate, is seen by the FDA as its client instead," he said.

He called on President Bush to appoint a new head for the agency. Lester Crawford has been acting commissioner of the agency.

In the FDA statement, Crawford said the FDA initiated and paid for reviews of Vioxx and antidepressants after those drugs had hit the market. "That is evidence the system is working," Crawford said.

Critics contend the agency ignored risks in both instances, then intimidated its own reviewers when they pointed to safety concerns.

In October, the FDA ordered that all antidepressants carry warnings that they "increase the risk of suicidal thinking and behavior" in children who take them.

The FDA's statement disturbed lawyer Andy Birchfield, who is evaluating thousands of potential cases against Merck on behalf of injured patients.

"How can they see that type of problem and look back and say 'We did everything right'?" Birchfield said. "When they're not willing to recognize mistakes, we have no hope for them voluntarily taking measures to correct the situation."

Crawford's statement did not mention Graham by name, but suggested that the reviewer was a maverick who did not follow agency protocol.

Graham was lead author on a research project that studied the records of almost 1.4 million Kaiser Permanente patients, including 40,405 treated with Pfizer's Celebrex and 26,748 treated with Vioxx. The study found that high doses of Vioxx tripled risks of heart attacks and sudden cardiac death.

Vioxx was responsible for an additional 27,785 deaths from heart ailments from 1999 to 2003, Graham concluded.

He has told congressional investigators that superiors pressured him to soften his conclusions.

Crawford said in his statement that the reviewer "voluntarily chose to revise his conclusions, and he did so, in his own words, "without compromising my deeply held convictions.'"

Deadly Effects of Ozone Pollution

(Ivanhoe Newswire) -- Changes in ground-level ozone were associated with an increase in deaths in many U.S. cities, according to a study from the ongoing National Morbidity Mortality and Air Pollution Study at the Johns Hopkins Bloomberg School of Medicine in Baltimore.

Ground-level ozone is a pollutant in Earth's lower atmosphere that is formed when emissions from cars, power plants, and other sources react chemically to sunlight. This is different from the stratospheric ozone, which is higher in the atmosphere and protects the Earth from ultraviolet radiation.

The risk of death was similar for adults of all ages and slightly higher for people with respiratory or cardiovascular problems. The increase in deaths occurred at ozone levels below the Environmental Protection Agency clean air standards. The average daily ozone level was 26 parts per billion, and the EPA's maximum for ground-level ozone over an eight-hour period is 80 parts per billion.

For the study, researchers examined the total number of non-injury-related deaths and cardiovascular and respiratory deaths in the 95 largest cities in the United States from 1987 to 2000. They also accounted for other variables that could impact mortality rates.

Researchers calculated that a 10-parts-per-billion, or almost 35-percent, reduction in daily ozone could save nearly 4,000 lives throughout the 95 communities in the study.

"Our study shows that ground-level ozone is a national problem, which is not limited to a small number of cities or one region. Everyone needs to be aware of the potential health risks of ozone pollution," says Francesca Dominici, Ph.D., senior study author.
