Heartburn Drugs Linked to Diarrhea

CHICAGO (AP) - Holiday revelers beware: Seasonal indulgences like eggnog and fruitcake might give you heartburn, but the acid-fighting medicine you take for relief might lead to something worse, researchers say.

People on popular prescription heartburn drugs - Prilosec, Prevacid and Nexium - seem more prone to getting a potentially dangerous diarrhea caused by the bug Clostridium difficile, new research shows. C-diff, as it's known, can cause severe diarrhea and crampy intestinal inflammation called colitis.

Dr. Sandra Dial and colleagues at McGill University in Montreal examined data on more than 18,000 patients in the United Kingdom from 1994 to 2004. During that time, 1,672 cases of C-diff were diagnosed, and the numbers increased from less than 1 per 100,000 in 1994 to 22 per 100,000 last year.

Patients with prescriptions for powerful acid-fighters called proton pump inhibitors, which include Prilosec and Prevacid, were almost three times more likely to be diagnosed with the bug than those not taking the drugs. Those on less potent prescription drugs called H2 receptor antagonists, which include Pepcid and Zantac, were two times more likely than nonusers to get C-diff infections.

The widely used and heavily promoted drugs reduce levels of gastric acid that can keep C-diff germs at bay.

Dr. L. Clifford McDonald, a researcher at the federal Centers for Disease Control and Prevention, said proton pump inhibitors recently were implicated in a C-diff outbreak at a hospital and nursing homes in Maine.

"It's not surprising in my mind that there could be some association" with acid-fighting drugs, said McDonald, who was not involved in Dial's study. If there is, "I do think it would be very important because, boy, everyone and their brother seems to be on them."

Most study patients hadn't been recently hospitalized and weren't taking antibiotics, which both can increase risks for C-difficile infections.

Also, most patients hadn't been diagnosed with ulcers or acid reflux, so it's possible many simply had heartburn, Dial said.

"Heartburn in and of itself isn't dangerous," and can often be treated with less potent drugs, Dial said.

Her study appears in Wednesday's Journal of the American Medical Association.

A co-author is a consultant for AstraZeneca PLC (AZN), which markets Prilosec and Nexium, and Altana Pharma, which makes and markets another prescription heartburn drug, Protonix, in Europe. A spokesman for Wyeth, which markets Protonix in the United States, said the company hadn't seen the research and declined comment.

AstraZeneca spokeswoman Cindy Callaghan said patient safety is the company's top priority and that the findings are not the final word.

"Further research is needed in this particular area to determine the validity of a potential link," she said.

C-diff bacteria historically have been found in patients on antibiotics or with underlying illnesses, especially those in hospitals or nursing homes, but infections increasingly have been reported in the community.

Doctors think the growing trend is due in part to overuse of antibiotics but the new data suggest overuse of acid-fighting drugs may be another reason, said Dr. Michael Brown, a gastroenterologist at Rush University Medical Center in Chicago, who was not involved in the study.

The drugs are popular because they are so effective at fighting stomach acid, and are generally very safe, Brown said.

Brown said short-term use of potent acid-fighting drugs for occasional over-imbibing is unlikely to increase infection risks in otherwise healthy people, but that the results suggest doctors and patients "have to think twice about using such heavy acid suppression" over the long term.

Appendicitis Diagnosis Drug Removed

WASHINGTON (AP) - A fairly new drug used to diagnose appendicitis was pulled off the market Monday, after the government linked it to two deaths and other life-threatening side effects.

The Food and Drug Administration ordered that NeutroSpec, an imaging agent, be taken off the market - and warned health workers to immediately stop using any of the drug still on their shelves.

Manufacturer Palatin Technologies agreed, saying it would suspend sales and recall past shipments.

There is no risk to anyone who already received NeutroSpec, the FDA stressed. The allergic-type reactions occur within minutes of the drug's injection.

NeutroSpec carries a radioactive tracer to white blood cells and binds to them. When standard tests can't definitively diagnose appendicitis, doctors can use the drug and a radioactivity-spotting camera to tell whether white blood cells have clustered in the appendix, a sign of infection there.

In studies, NeutroSpec was administered to 523 patients with no serious side effects, FDA said, leading the agency to approve the drug as an aid to diagnosing appendicitis.

But since sales began in September 2004, FDA has learned of 17 patients who suffered life-threatening side effects, including heart failure, as soon as the drug was injected. Two died.

Most of the side effects occurred when NeutroSpec was used to diagnose infections other than appendicitis, FDA said. While so-called off-label use is legal, doctors have less information about potential side effects than when they use drugs in an FDA-approved manner. Also, many of the side effects occurred in patients with diseases, such as diabetes, that may have made them more vulnerable, FDA said.

The agency is investigating the risk, and plans to convene a meeting of its independent scientific advisers early next year to debate if additional safety measures would allow NeutroSpec to return to the market.

LENA'S COMMENT: Sadly more drugs are being removed as unreliable to down right deadly medication in the last 10 years than all the years before. That sounds good, but sadly people die and/or become extremely ill before it happens. Drug companies are rushing to make megabucks with prescriptions and are presenting tainted studies to get them approved... Beware of taking chemical prescription or OTC drugs if they are less than 10 - 15 years on the market...

The Christmas Fruit 

This time next week this fruit will make it's appearance in more homes than usual. Kind of traditional fruit...

It might just be because they are coming into harvest or it has been passed down from generation to generation as the fruit to put in every child's stocking. Whatever the reason that they occupy a place of honor for this time of the year that orange ball is healthy, nutritious and a very enjoyable food.

An orange is more effective than dark-green, leafy vegetables in increasing serum concentrations of retinol and beta-carotene.

There are various kinds of oranges.

The orange has a sweet with a slightly sour taste and is commonly peeled and eaten fresh, or squeezed for its juice. The outer-most layer of the rind is called orange zest, and it has a similar flavor with a bitter tang to the inner part of the orange. The rind is very high in nutrients and can be used in cooking, cleaning or eating. When the oil is removed from the orange rind it is a very effective non-toxic cleaning and polishing agent. Orange oil is used as a wood treatment for preserving and naturally beautifying it.

The fruit originated in India or Vietnam, depending on who you listen to. It was called na rangi in Sanskrit. The na rangi or naranja was translated as "norange", and in English usage a norange was back-formed into the more acceptable an orange. The same thing happened in French and Italian, but in Spanish it is still naranja.

A number of varieties of orange are now cultivated in many parts of the world, usually in warmer climates. The sweet navel orange - Citrus aurantium - was first grown in Spain, and has become the most popular variety. The sweet orange will grow to different sizes and colors due to local conditions, most commonly with ten slices inside.
 

The Valencia or Murcia orange is one of the sweet oranges used for juice. It is a late-season fruit and is the most popular orange when the navel oranges are out of season.


The blood orange has streaks of red in the fruit, and when squeezed the juice is often reddish. The blood orange, with it's sweet, deep red colored flesh, is thought to be the result of a mutation that occurred in 17th century Sicily. In most varieties of blood orange even the pulp and skin is slightly sweet, and not bitter.

The mandarin is similar, but smaller and sweeter. The mandarin orange is considered a native of south-eastern Asia and the Philippines. It is most abundantly grown in Japan, southern China, India, Mexico and the East Indies, and is esteemed for home consumption in Australia. Mandarin is a group name for a class of oranges with thin, loose peel, which have been dubbed "kid-glove" oranges. These are treated as members of a distinct species, Citrus reticulata Blanco. The name "tangerine" could be applied as an alternate name to the whole group, but, in the trade, is usually confined to the types with red-orange skin. In the Philippines all mandarin oranges are called naranjita. Spanish-speaking people in the American tropics call them mandarina. There are a multitude of different named Mandarin oranges.

My grandchildren's favorite breakfast is

Orange French Toast;

Ingredients;
3 eggs beaten
1/8 teaspoon ground cinnamon
1/2 cup fresh squeezed orange juice
1/4  teaspoon orange zest
1/2 cup soy milk
1 Tablespoon melted Butter
6-8 Whole wheat bread slices
 
1) Beat first six ingredients together in bowl until well mixed.
2) Heat griddle.
3) Dip bread slices in mixture coating both sides.

4) Cook on butter coated or non-stick griddle until lightly browned. If you like your French toast slightly sweeter lightly sprinkle with powdered sugar. For a more cinnamon taste sprinkle with sugar and ground cinnamon before serving. (I keep a salt shaker with cinnamon/granulated sugar mixture on hand for this purpose. 1/8 teaspoon cinnamon to 1/4 cup sugar.)
 
Serve hot with fruit or syrup of choice. (Our favorite is with sliced peach or sliced strawberries)

Old Curative Gets New Life at Tiny Scale

Silver, one of humankind's first weapons against bacteria, is receiving new respect for its antiseptic powers, thanks to the growing ability of researchers to tinker with its molecular structure.

Doctors prescribed silver to fight infections at least as far back as the days of ancient Greece and Egypt. Their knowledge was absorbed by Rome, where historians like Pliny the Elder reported that silver plasters caused wounds to close rapidly. More recently, in 1884, a German doctor named C. S. F. Crede demonstrated that a putting a few drops of silver nitrate into the eyes of babies born to women with venereal disease virtually eliminated the high rates of blindness among such infants.

But silver's time-tested - if poorly understood - versatility as a disinfectant was overshadowed in the latter half of the 20th century by the rise of antibiotics.

Now, with more and more bacteria developing resistance to antibiotic drugs, some researchers and health care entrepreneurs have returned to silver for another look. This time around, they are armed with nanotechnology, a fast-developing collection of products and skills that helps researchers deploy silver compounds in ways that maximize the availability of silver ions - the element's most potent form. Scientists also now have a better understanding of the weaknesses of their microbial adversaries.

One of the urgent goals is to prevent bacterial infections that each year strike 2 million hospital patients and kill 90,000, according to the Centers for Disease Control and Prevention. Such infections are usually treated with large doses of antibiotics and sometimes with repeat surgeries. They cost the health care system roughly $4.5 billion annually, and the challenge is growing with the spread of drug-resistant microbes.

The latest advance for silver therapy comes from AcryMed, a small company in Portland, Ore., that has invented a process to deposit silver particles averaging about 10 nanometers - less than a thousandth the diameter of a human a hair - on medical devices. AcryMed's first customer, I-Flow, makes a silver-coated catheter that pumps painkillers into the wounds created by surgery.

I-Flow got federal regulatory clearance on Dec. 2 to sell the device and has already begun shipping them to customers. The nanoscale particles have so much surface area to react with the microbes, in relation to their volume, that small concentrations are effective antiseptics.

"The equivalent of a teaspoon of silver in a seven-lane Olympic-size swimming pool is enough to do the job," said Bruce Gibbons, the microbiologist who is AcryMed's founder and chief executive.

AcryMed hopes to reach agreements with catheter companies larger than I-Flow, including the makers of urinary catheters, the most common breeding ground for hospital infections. Nano-scale silver could also eventually make its way onto permanently implanted devices like silicone breasts, artificial hips and knees and pacemakers.

The term nanotechnology is derived from the nanometer, one-billionth of a meter. Nanoscale materials often exhibit unusual structures and behaviors compared with bulkier quantities of the same material.

AcryMed began developing wound dressings with large-scale silver particles in the 1990's. It adapted its process to make smaller and smaller particles when medical-device makers asked it for silver coatings. As the particles shrank to nanoscale dimensions, they became so highly reactive that AcryMed was able to bind them to virtually every glass and plastic material it tested.

As has often been the case in nanotechnology, AcryMed is rushing to get products into the market long before the nano-scale phenomena it is exploiting are fully understood. It is not entirely clear, for example, how silver kills many bacteria at the diluted concentrations considered safe for medical use.

AcryMed also admits that it cannot fully explain the forces that produce the surface-binding performance of its particles. It is a crucial trait, though, because surface coverage that is even and thorough blocks the formation of a thick carpet of bacteria known as biofilm.

Preventing the build up of biofilms, which eventually release large masses of free-floating bacteria, is in turn a key to avoiding infections, according to Dr. Dennis Maki, a professor of medicine and the head of infectious diseases at the University of Wisconsin Medical School and Pubic Health.

LENA'S COMMENT: New information on an old story... I've been telling you Silver Water is great for years and now the media in some areas of the country is saying it... Even though the media is a bit ambiguous and a bit premature on this particular silver it will help spread the word on silver being a viable antibiotic alternative!!!

DuPont to Pay $16.5 Million for Unreported Risks

WASHINGTON, Dec. 14 - The Environmental Protection Agency said Wednesday that it had reached a $16.5 million settlement with DuPont, which it had accused of failing to report information about the health and environmental risks of a substance used in making Teflon and other plastics.

The settlement includes $10.25 million in fines, which the agency says is the largest administrative penalty it has ever imposed, and a commitment by DuPont to spend another $6.25 million for two environmental projects in lieu of more fines.

"This settlement sends a strong message that companies are responsible for promptly informing E.P.A. about risk information associated with their products," said Granta Y. Nakayama, assistant administrator for the agency's Office of Enforcement and Compliance Assurance.

The agency said DuPont had withheld information about perfluorooctanoic acid, a chemical known as PFOA that is used in processing high-performance plastics, from residents near the company's plant in Parkersburg, W.Va., where the chemical contaminated local waters. The agency said the company had information on the chemical's potential risk as early as 1981 but did not report it, as required by the federal Toxic Substances Control Act.

Stacey J. Mobley, DuPont's general counsel, said the company had interpreted the reporting requirements differently but had agreed to the settlement "to get this case behind us."

Under the agreement, DuPont was not required to admit liability, a standard arrangement in cases the agency is eager to close.

While Mr. Nakayama described the size of the settlement as "appropriate" when weighed against the costs of litigation, the Environmental Working Group, a research organization, described it as woefully inadequate for DuPont, the nation's third-largest chemical company.

"This fine, at the very least, should have prompted DuPont to apologize to the public for its actions," said Ken Cook, president of the group.

The information that the agency accused DuPont of withholding came to light during a 2001 class action lawsuit against the company, filed by residents living near the West Virginia plant who claimed that the chemical had contaminated local waters. The company settled the case last February for more than $107 million.

After Mr. Cook's group brought the information to the E.P.A., an agency investigation led to eight charges against the company that largely focused on studies of PFOA in blood, water and rats.

Officials from the agency and DuPont said that so far there were no studies proving that PFOA was a significant health hazard to humans.
